Should I be concerned about my testicle pain yet?

Have started having pain in the right and left testicle, cant recall it hurting both at the same time. My wife and I have checked all over; no lumps, swelling, or shrinking anywhere in the sack. I can also urinate and have orgasms 100% pain free. I have no insurance so I can't afford seeing a doctor whenever I want, should I schedule an appointment? It's not constant pain and there's no nausea or vomiting either. Im 29 if that helps too.

Do you have long periods of arousal  without ejaculating, that seemed to be my problem with similar pain you mention, I would have maybe 2 or three days of prolonged arousal for hours at a time,e dging to almost climax,  and think maybe a build up of semen in tubes was the cause of the pain, but when i masterbated to climax the amount of heavy lumpy semen that came was instant relief, and the amount that came was loads more than a normal cum,  maybe not your problem tho, but thought it worth a mention.....I now know when to cum rather than have long sessions of play and edging to near climax, then can get rid of pressure in tubes which seemed to be the cause of such pain, I never had professional advice on the matter as i was too shy to explain my sexual habit..and fear of physical examination .

I get severe testical pain  ,if i have been aroused a lot and not ejaculated ,and when i get the pain i masturbate and ejaculate and the pain eases almost right away,  maybe you have the same problem

Testicular pain can be because of a lot of things.  Like a huge list!  Here is the list:  https://www.mayoclinic.org/symptoms/testicle-pain/basics/causes/sym-20050942   Underlying issues like having diabetes is on the list, torsion, neuropathy, hydrocele, prostatitis, injury, etc.  Lots of reasons.  It's hard to say what is going on with you.  I know an awful lot of doctors take cash now and will charge less for an uninsured cash paying patient.  There are also teledoctors although you will likely need an in person exam.  And with the corona virus, it's a bad time to go to the doctors.   It's like a dull ache all the time in both testicles unless urinating or having an orgasm?

Not all the time, i have several hours without any pain. When it starts it's just a dull achy pain. Urinating and sex doesn't ease pain, but i read that some reasons for testicle pain also causes an increase in pain during urination and orgasms but Im not getting that effect.
Can you recall any injuries or trauma to the area?

Since you don't have health insurance, I'm assuming you're in the US. Can you look up your county's health department and call and see if they have any free or low cost clinics for people who have no insurance? Ask about income requirements, because some have those even if you don't have insurance.

Also ask them what their covid19 protocols are for non-related appointments.

Have now started getting pain in lower back and chest with a shortness of breathe.

Getting scared it's something serious but with my shortness of breathe no doctor will look at me because of covid-19.
Shortness of breath from the testicular pain or do you have respiratory symptoms?  People do still need emergency care even with covid-19 going on and if you have serious shortness of breath, you need to take care of that.  Are you having anxiety though? You sound quite panicky.  The issue with your penis is likely not related to shortness of breath.  They should have an ER phone number for you to call or urgent care numbers for you to call and they are offering telemedicine to people to minimize people coming in that don't have to.  So, touch base with someone locally.  I'm wondering if you aren't having a something panic/anxiety related but chest pain and shortness of breath always needs to be checked out in case it is cardiac related.
