US dated 4/8/08:

Right lobe measures 5.3 x 2.0 x 1.4 cm, left measures 5.0 x 1.3 x 1.6 cm

On the right, heterogeneous echo texture is seen without a discrete focal lesion.  On the left, similar findings are present.  Impression:  Heterogeneous thyroid upper limits of normal in size.  No focal suspicious lesions identified."

My blood work is normal : Free T4: 1.45; TSH: 1.04; TPO ab: <10.  No T3 done

I guess I now have a name for the thyroid hell I've been wading through....

The isthmus appears normal.  Right thyroid lobe dimensions currently are 1.6 x 2.0 x 4.8 cm compared to prior dimensions of 1.4 x 2.0 x 5.3 cm.  Left thyroid lob dimension currently are 1.5 x 1.5 x 4.8 cm compared to prior dimensions of 1.3 x 1.6 x 5.0 cm.  These values are not considered significantly changed.

On the right, there is slight diffuse heterogeneity of the echogenicity without a focal lesion.  On the left side, there is diffuse heterogeneity, also without focal lesion.

Impression:  Stable, mild thyromegaly without focal thyroid lesion.  Findings are consistent with a mild pattern of multinodular goiter.


NOW --- can you have a multinodular goiter without a nodule??  Hmmmm... WHAT??  - I AM CONFUSED.  If I don't have a nodule, what's the **** beside my trachea?  Hmmmm..........

The multinodular goiter MUST have discrete  NODULES!! Someone who wrote report must go back to school!!!
If I use old school terminology dated back to 1971, it should read: “first degree of thyroid enlargement”, as the length of lobes are longer then distant phalange of your index finger.
About 20 % of the population  used to have this finding.
