Changes in my son behavior

My 7 years old son has ADHD but lately they has been a change in his behavior:
1. Increase talking to himself for hours at a time and only stopping when he is sleeping or eating.
2. Playing with his feces in the bathroom and putting on everything.
3. Major temper outburst where he has to be physical remove for classroom.

I am doing my best to understand and figure out what is going on in school, his after school program, talking to his teachers and friends. Even looking at our home life to look for any major or little changes that might be effecting his behavior change. Please advise on what I can do?

Is playing with feces and poor self care necessarily a sign of ADHD?  I'm sorry he is having so many issues, is he on meds, has he been evaluated lately?  I would call the dr. and ask, esp about the feces.

Sometimes symptoms will change with age.
My niece has ADHD...I have ADD.
She is dirty and disgusting too. She will not flush the toilet,wash her hands,comb her hair...she is now 13.
She just started having her period and things are getting worse. Her parents put her on the medication but it doesn't help any. The other girls at school pick on her because of her filthy habits. It's very sad.
Her parents are taking her in to have her meds adjusted. We'll see if that helps.
Nothing in her life had changed previous to this...just age.
