Stubborn doctor isn't helpful with dosage issues

I am a military wife so I see a primary care physician on a military base. My doctor doesn't pretend to know what she's doing when it comes to my ADHD medication. She does, however, pretend the heck out of knowing what to prescribe me for my anxiety issues. But that's a different issue.
I'm currently prescribed Adderall XR 60 mg's a day but I could honestly sleep straight through it now, and when I try to explain this to my doctor she gets flustered and says "well 60 is a lot to be taking." Which I agree with totally but whenever I try to discuss other medication options she looks even more nervous. I don't have the ability to see a psychiatrist all the time because I have two young children at home and my husband has to work. I don't have the money to get a babysitter every time I need to go to the psychiatrist. I also feel like if my doctor would just do some research and actually listen to me that we'd be able to work out something better.
I have another appointment on the 25th for a refill and I don't know what to do to get her to listen..

Has anyone had this sort of issue before? Going through it now? Anything? Any advice would be more than greatly appreciated!

It is a sound practice to look up interactions on the internet whenever you are prescribed a new medicine. I do this religiously and you would be surprised how often doctors have prescribed medications that interact with each other. I would say this happens more often than not. The problem usually arises when a specialist prescribes for a condition outside his speciality.

   Quick thought cause I need to find where I saw the info.  But I know that some anxiety meds can partially damp the effects of stim meds.  For example, "Wellbutrin can block the pathway for Dex, only moderate, but if the person is s slow burner that moderate blockage of 2D6 could spell the difference."  If you don't mind me asking, what anxiety meds are you on and I will see if that effects the stim med.
    Oh, the other more important question is - does the adderal make you sleepy?  Or basically does it just not affect you.  There is a huge differance between the two affects.
   Found it.  ": Prozac and Paxil both significantly block the pathway for the metabolism of Adderall and all the amphetamine products.  So if you are using either of these two with adderall - that could be part of the problem.
