Neuro symptoms

Since I last posted, I have had a negative LP and negative emg/ncv on extremities.  Extensive lab work up negative for MG, autoimmune diseae and heavy metal poisoning. Negative CT c/a/p.  I have now lost 65 pounds. I have vibratory sensation and tremor in hands. Vibrating in feet.  Widespread twitching. Perceived neck weakness. Diagnosed with sleep apnea with normal abg and pft's.  Tongue quivers and mild trouble with speech. Next neuro appointment not til end of November.  Any ideas?

Went for appointment at Cleveland Clinic.  Had emg of left arm and leg.  Showed motor axon loss changes in left foot.  Ncv was normal. They did not check cervical paraspinal area or bulbar.  Should that be done in light of neck neck weakness and trouble swallowing etc.  Worried this could be bulbar onset ALS??

Thank you for taking the time to respond.  I failed to mention I recently had another MRI of the head, cervical and thoracic spine that was essentially normal.

Hi again!

This is likely to be associated with peripheral neuropathy/ myopathy or physical issues. Pattern of the delayed conduction may provide a base for further testing. Detailed imaging studies may help diagnose/ rule out areas of demyelination. Further testing may be required to look for possible secondary cause of the neuropathy/ myopathy as well; and accordingly a further management plan may be chalked. It is best advised to discuss this in detail with your treating neurologist.

All hormone levels, vitamins and glucose are normal.  Repeat emg/ncv shows delayed conduction and recruitment??

Hi there!

Since most of the tests have not showed up anything I would suggest going in for a detailed imaging study next where other neurological condition may be evaluated for, before we start to consider non-neurological causes of your symptoms like micronutrient deficiencies, hormonal/ endocrine/ autonomic disturbances like thyroid imbalances, diabetes, myopathy, metabolic disorders etc.
