my dog is in heat. she has a balloon thing protruding from her vaginal area the size of a plum. she has never been penetrated . is this normal?

She really needs to be examined by your veterinarian. It does not sound normal.
I agree with ireneo, my dog did the same thing when she went into heat. Get her to the vet.
Sounds like a prolapse. She should be seen by a vet to avoid injury. Don't delay.
