Yea it doesn't matter what shoes she wears, even the ones that are a little lose it still hurts. It hurts more when she walks on slop or in elevated environment. I didn't pay too much attention to when she was walking on the beach to see if it was more or less uncomfortable, she feel it's more or less the same as walking on normal grounds. Turning and change direction doesn't significantly increase the pain but she can feel it being pressured.
Will any orthopedist work and give us a straight answer? should we get an X-ray before we go?

The pain comes no matter what shoe style she is wearing? I've known the wrong pair of shoes to cause severe back pain.
How about walking barefoot up and down a beach, especially a beach with a slope. As she changes direction, she changes her weight distribution. I wonder if this changes her symptoms?
Regardless, I'd suggest that she see an orthopedist who specializes in hips, knees, ankles -- the lower extremities. This would be a good place to start, and even if this is a back problem, the orthopod will get it sorted out.
