How can I relieve my grandfather's back problems

I've been taking care of my grandfather for the last several months after he fell and fractured his l1 and l2 vertebrae. His back has healed, but the pain is still too much for him to walk. Painkillers are ineffective, our local physicians have proven incompetent or just unwilling, but we had a massage therapist helping him for the last 3 weeks, and we've finally made progress. He can walk short distances holding on to things. However, unless something is on tv, he won't leave his bed. The real problem comes from both stubbornness and a life of relative inactivity. He refuses to eat healthy, despite suffering from a host of medical issues, and has always just sat in a chair for as long as my mother has been alive. He also refuses therapy outside of a very cramped home. So it's up to me and my uncle. But I've gone these several months without work, and placed my life on hold. I can't wait around anymore. The question is how do I help him? Does he need to fight through the pain and walk about, or does the answer lies elsewhere? Again, pain pills and patches are ineffective, no therapy, and he's extremely stubborn. He's much like a child in some regards.
