What to do with numerous problems in school, home and community?

I have a almost 5 year old son.  He has behavior problems everywhere.  He hits, kicks, destroys his own and others property, runs away from teachers and myself (sometimes into the street or a busy parking lot, he has been found in empty cars and closets at school.  I just do not know what to do.  I have tried spanking, time outs, a reward system, taking things away, he just does not care.  What can I do?  he is also a very scared child in that he will not go to bed because of nightly nightmares, he doesn't nap.  He is very smart, above average in just about everything, reading, math, general knowledge.  I also cannot take him to friends houses because he has a tantrum when we leave and someone has to sit in the back seat with him because he takes off his seat belt and tries to get out of the car when we are driving. We went through the school system to try to get him help but they stated that because the problems do not effect his knowledge they will not do anything.  I am starting him with a therapist at children's hospital, but we will see how that goes.  Please help me find some way to deal with this personally and with him.  I cry almost on a nightly basis because I just don't know what to do anymore.  I am disabled with neck, back, and hip chronic pain so I cannot run after him or carry him around when he is having a fit so I always have to have someone with me.  I have no life outside of him.  Please give me some advise on what to do.  Thank You for reading and responding.

Had you not already arranged the involvement with the mental health clinician, i would have advised you to do so. Your son's behavior indicates a serious level of emotional disorder.

Wow..no worries your not alone...Something you need to do is weigh out your options...Who else can better care your child then you? Who has more
Patient for your son you...It all boils down to you, you can not depend on others cause no one can love him like you. I can tell you to put him in a state hospital, a mental institute, or drug him up but honestly our kids need their mom and dad.

Get motivated, think outside the box, there most be
Other moms dealing with the same thing as you...Reach for the stars for your son with some love, a support system you can do it. Ps...i have a son with issues as well and trust me we get a lot of bad and good attention...
