Chronic Perineum / Anal Itching.

Hi there. I'm new to all this and have never asked a question online before. I'm going to be very specific with my details so if you're squirmish- DON'T READ!
Right, I'm a 33 yr old female.
I've suffered with perineum itching for years, since a young child in fact. It's so itchy - I've always scratched it until it's bled and you can feel the tears in the skin.
I Have always put it down to allergies to toilet paper. I can't use coloured, scented, aloe, shea ect. I have to use a branded plain white which eases on occasions.
I also developed a haemmorhoid on my anus. It's been there for a few years. I do put ointment on it when it 'flares' up but it never disappears. (Is that normal?)
I'm not a haemmorhoid connoisseur so I presume it is a haemmorhoid. On my anus ring it's like a flappy piece of skin, bloody itches like mad when I go to the loo!
I know I have an itch on the anus due to the haemmorhoid but why is my perineum so itchy. I've also noticed a small pea sized lump just to the left of my perineum, the lump is inside rather than growing on the outside if that makes sense?
The lump, my perineum and my anus is just in tatters. I feel so miserable it's getting me down. I dread going to the toilet as even a tiny amount of urine splashing on my perineum stings like crazy.
I really need to see my gp but as you know it's the embarrassment of getting my nether regions out!
Just wondering if anyone can shed some light?
