I don't know if is the picture, but I see white spots in your glans also. I had redness as you and white spots and medication didn't work. I had it for 1.5 years and it was worse in the summer. After visiting many doctors and several tests the last one I went was an urologyst and told me I had recurrent balanitis and was caused by my foreskin (was uncut) cus was making the perfect moist and warm conditiions for bacteria and fungus to grow. I can't tell in the picture if you are circumcised or not, but I went ahead with the surgery and the balanitis went away with my foreskin, now my penis head looks a healthy color and no more red and white spots. Operation happened 3 months ago and no sign of redness anymore. Consult a male urologyst he will give you a better answer.  Thanks   good luck

MEDICAL PROFESSIONAL
Hello,
Without examination, confirmation of a diagnosis is tough but it can be contact dermatitis, rash due to friction rub with clothes, dermatitis (atopic, contact, allergic),excessive sweating causing sweat dermatitis or balanitis(due to an underlying infection (bacterial and fungal as well as other organisms) .
You may try calamine lotion on the rash. You should also maintain hygiene and try to keep the area clean and dry, wear loose-fitting cotton clothing. Also use an antibacterial soap or mild soap like dove. Avoid any kind of cosmetics. You can also use a barrier cream with zinc oxide paste or calamine lotion or a mild steroid cream like dermacort on the rash. If the symptoms are severe then please consult a dermatologist.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.
