Safety in bed

When my girlfriend and I have sex we always use a condom and I always pull out before I ejaculate just to be safe, but I have two scenarios that I'm extremely worried about her potentially being pregnant.

1) Last saturday when we had sex I started to put the condom on and when i realized it wasn't unrolling right i flipped it over and put it on correctly, even though I was wearing protection I pulled out to be on the safe side. I didn't pre-*** before I put the condom on wrong.  I also masturbated 2 times within 5 hours before we had sex and as a result we had sex for approximately an hour.

2) This friday when we had sex I was wearing a condom and got off early, I put on a second condom and started going again. I didn't spill any semen around her but my penis was sticky after I replaced the condom.  As a sidenote I masturbated once 5 hours before we had sex.

What are the chances she could be pregnant?

Chances are slim to none.  Semen doesn't live outside the body long.  Even it was able to make the trek inside your girlfirend the chances of it travelling through to her cervix, uterus and then to an egg if she is ovulating are very slim.
