Missing ovary!

Can someone Help me! To begin my story, I was hospitalized with polycystic ovaries in 96,97,98 with extreme pain. I stayed overnight to manage pain and no surgery was performed..I have records indicating good size ovaries...

For a long as know, I have been suffering from bad menstrual cycles only on day1. I had my first laproscopy in 2004 to diagnose endometriosis and it was rulled out due to it being very minimal.Both ovaries looked good. The last couple of years, the pain is getting so intense. I have experienced extreme pain only located very low on my right side on Day1, vomiting, shakes, fatigue, tingling in arms and legs, aches and pain thorughout my back and whole body and becoming disorientated.
In May of 08, I had  enough I went in to the doctor for an ultrasound and they couldnot locate my ovary on the ultrasound do to the fecal matter in my intestines..I Later had my last laproscopy from a different doctor and he stated that I did not have a right ovary but I had a tube and ruled out endometrosis..My questions are Can an ovary just disappear without signs or complications? Could it have ruptured and I survived without infection etc.?If not, where could it be? And last, since endometrosis is ruled out what I can do, to survive day1 of my period? I have been tring to conceive for 15 months now. Started clomid and estrace this cycle.My only concern is blowing my only good ovary..Please I need medical advise.

Thank you so much for your response..I do have surgical records indicating I had a right ovary from previous surgery.The last surgery showed no signs just a partially blocked fallopian tube on the right..
A little confusing but do you think a doctor while existing made a mistake and didnt document it...Where could it have gone..I
This stresses me out everyday and those stresses are probably preventing me to conceive.No doubt
I had tests done for my FSH and follicle count all are good...I have over 100,00 eggs left in my left ovary. Tube is clear progest is good..I started 25mg clomid and my web research shows they should increase by 25mg every month if you dont conceive and exam you pre and post ovulation..I do not have a trans vaginal ultrasound set up..Just a little comcerned about blowing my only good ovary...He does mention several times let just do IVF...I feel like he is throwing the towel in way too soon...

Hi!
No, ovaries do not disappear--even 80-year-olds have little tiny ones.
It is possible that you were born without one, but it sounds like you have at least a few ultrasounds showing the ovary was there.

It is EASY to miss an ovary on ultrasound, so that is very explainable. The laparoscopy is a little more tricky to try to guess at.  Do you have your actual operative report? Did your doctor take any photos?

What I would do if you came to see me is:
1. Get all your records together and review the notes.
2. A day 3 FSH level (or some other/additional test of ovarian reserve)
3. A transvaginal ultrasound with a follicle count on about day 12 of your cycle.
4. A TSH and Prolactin level and semen analysis and HSG (if not yet done)
5. Start you on Glucophage if tolerated right away given your history of PCOS.
6  Stop clomid if you have taken more than 3 cycles and consider switching to Femara
   for another 3 cycles vs injectables.
7.Consider adding Intrauterine insemination
8. Refer you pretty quickly after a MAX of 6 cycles to see a reproductive endocrinologist before wasting any more of your time and money.

Don't dink around too long with the same doctor, or different ones who are giving you conflicting stories and no results--find the best one you can access and get yourself in there!
