Unprotected Vaginal Sex with a Prostitute

One week ago, I got drunk have an unprotected uncircumcised insertive vaginal sex with a prostitute using Viagra. 3 Days post exposure, there's a little pain and see redness on my penis suspecting I have had gonorrheal infection. After searching the internet, I took a single dose of 2g of Azithromycin. 2 days after I took Azithromycin (Day 5 post), I experienced diarrhea for 2 days, it has stopped but I still feel stomach discomfort. On day six, I can see little red spots appearing on my body.

At day 2 post exposure, I have had a lab test done, testing for syphillis, gonorrhea, chlamydia, and HIV. HIV came out non-reactive since my reason for testing this is to know if I ever had any HIV prior to exposure since I have a wife and a small daughter (1.5 years). I tested positive for gonorrhea which the lab test using gram stain.
Then, today (Day 7 post exposure) I visited an std doctor, the doctor told me that my infection is not gonorrhea but it is a non-specific urethral infection. Gave me an IV shot and gave me 2 kinds of unkown tablet to eat 3 times a day. I have been deeply worried to contract HIV because I do not know what to tell my wife and leaving both my wife and daughter with risk of not only HIV but also STD that I cannot test and still in the window period.

Worried about the diarrhea and tiny red dots, I tested again today for HIV as well as Hep B and Hep C.

It is a huge risk, considering I have uretheral infection from the prostitute and I am uncircumcized with insertive vaginal sex.

But I wonder if you think I even have at least a 51 percent chance of not contracting HIV assuming that the prostitute did have one? The STD told me that the chances of me getting HIV depends solely on the prostitute. Also, I ready somewhere that HIV genotype in Asia is of E type which is more virulent than the ones in United States.

When should I get tested? And what is the time period for me to be tested for other STD and what other STD should I be tested for?

Hello Ghex, sorry to hear about the herpes - was this a doctors diagnosis, or after the result of a test or simply internet guessing? My feeling is the same as that of Dr Jose. The chances remain slim so I would repeat his message of keeping things calm and in perspective. I have no doubt that the stomcah symptoms were to do with the large doses of azithromycin. It is very probable that the azithromycin at that early stage would
satisfactorily kill syphilis, gonorrhoea, chlamydia, most types of NSU. The symptoms you describe are not consistent with HIV and not consistent with the timing of your exposure.

Thank you very much for your post and my sincere apologies for the delay to reply.  I am not sure why your post was missed.
I am also very sorry about all the stress and anxiety you are going through. I agree with the very kind contributor that has written a comment in your thread.  Gonorrhoea, Chlamydia and any other infection causing any kind of urethritis are extremely common and very easy to transmit.  Equally they are all very easy to treat with the appropriate antibiotics as you have been prescribed.  From the point of view of HIV infection, in spite of the added infection and that you are not circumcised, the risk for you from a single encounter of unprotected penetrative vaginal intercourse is relatively low and this is even assuming that the sex worker was definitely HIV positive.  You do not seem to know her actual status and statistically is more likely that she is negative.  Even if she had been positive, the risk for you would have been less than 0.1% or less than 1 in 1,000.  Therefore what you need to try to do now is to put things into perspective, stop looking so much in the internet and get a PCR test after 10 days, or even better the new Multiplex HIV1/HIV2/HepB/HepC PCR test, highly reliable after only 7 days.  I am not sure if this is available near where you live, but if negative this test will give you a practically conclusive result with that added peace of mind.  In the meantime you have to disregard all those symptoms that you have been having.  It is far too early to have any acute HIV sero-conversion and they are most likely linked to side effects of the antibiotics and your stress and anxiety.  I strongly believe that you are going to be fine, as you need to realise that it is much more difficult to contract HIV through penetrative vaginal intercourse than people think.

Thank you so much to all my friends here.

Here's the update:

I did the 6 weeks antibody test and the result is negative.

I also did a 6 weeks DNA PCR test which actually looks for the virus itself and thankfully the result is negative.

Did another 7 weeks antibody test and again thankfully the result is negative.

Because of the negative antibody tests and negative PCR tests, the doctor told me that I can be 100% certain that I did not have HIV.

Truly great to know that I am actually HIV-negative.

About the small bumps being visually diagnosed by the doctor I see here in this third world south east asian country:

I went to Singapore and did see two other dermatologists and an infectious disease specialist and all three of them told me that they are 99.99% sure that what I had was actually not genital herpes but it is fordyce's spots.

To be 100% sure that that the small bumps was not herpes, one of the dermatologist then took a sample from the skin and performed a biopsy on it and the result turned out to be fordyce's spots.

And I also did a 6 weeks antibody test for herpes and just got the result today that confirmed that I do not have any herpes and the doctor told me that I can be 80-90% sure that I do not have any herpes. Another even told me that the accuracy can be as high as 99%. But I will still wait for and do a 12 weeks herpes test to be 100% sure.

The first dermatologist that I see here in this third world country and scare the **** out of me and saying that I have genital herpes by his wrong visual examination and even told me that I can be 100% certain that I will get HIV if the girl that I had sex with actually has it is a really scary dermatologist. He even went so far in scaring me that HIV will be transmitted even by the smallest of microscopic abrasion during the sexual act. Which is totally not true. I cannot believe the quality of the doctor can be so poor in this part of the world.

Again, I truly truly appreciate all of your comfort, well wishes, and great support.
You guys are angels that help me go through this most difficult part of my life.

I am really thankful that all the results turned out to be okay.

I told my wife about it. She cried the whole night and I feel so sorry to her.
I really really wish that this lesson would not cost me my life.

I took Acyclovir tablets 400 mg, 5 times daily for 5 days.
And took a viral test to see whether I have HSV-1 or HSV-2, which will come out this Tuesday.

Wonder if anyone here can tell me weather Acyclovir can effect the accuracy of an HIV EIA antibody test?

Will EIA Antibody test be conclusive for 6 weeks? or 3 months? or 6 months?

Also, can genital herpes be transmitted from skin to skin contacts during outbreaks?

I can strongly relate to you situation as the same thing happened to me. Having a child and wife is the only reason for the high level of stress and guilt. You think of very bad things including your life and infecting the one you care about but please take this advice as I have been their and still some thing to get over. The doctors know what there talking about and have no emotional content to you. What this means is that you are getting the true facts. Like the 1 in a 1000 chance of getting HIV if the person was infected. That would mean that you would need to have unprotected sex with that person more than twice a day for an entire year to catch HIV. The other thing is that she may not have HIV. Your wife will ask questions soon about how you are cause you seem different because she knows you. To buy time I used to say that I was streesed from work as my excuse until you have your test done. It was a mistake and your human.

i am sure the doctor will reply soon but you need to calm down a bit. As you can see from the many posts from the Drs on this forum, the chances of getting HIV from a single encounter of unprotected vaginal sex are very low.

As for testing, the doctors can help you. Personally I wouldn't tell my wife until I had calmed down a bit and had more information about testing etc. Don't have unprotected sex in the meantime?

Try and calm down its easy to panic, we've all been there but you are suffering from anxiety attacks that probably are unrealistic compared to the actual risk

My friend you are so lucky!! I did the same thing as you but I got HIV from it! In some ways I'm lucky as I don't have a family, make sure you never do it again!

Thank you my friends, for your prayers! At least I don't have to explain to you guys how I'm feeling going through this!!! The opinions of the doctors here are very reassuring, but the feeling that there is hope after seeing you guys come out of this ok is even more reassuring!

Monday is my 6 week mark, I'm going to the doctor's to do another test, hopefully they will have some of the more advanced testing available so my result (which I'm on my knees praying every day and night will be negative!) will be more conclusive.

I thank both of you.  And this entire forum for all it's usefulness!  I received my PCR results just minutes ago, they were negative!!  The relief I feel is unexplainable.  And I had every symptom.  I had bumps, a huge rash on my chest, spots on my hands, a sore throat, and the worst night sweats of my life.  I am truly amazed and thankful for the second chance I've been given.

Congrats Ghex!  I'm glad to hear you had the negative results!  I've been following your story.  I've had all the symptoms, night sweats, chest rash, sore throat, and spots!  I've tested negative at 4, 6, and 7 weeks.  I'm almost at 8 weeks and yesterday took the PCR test and should get the results in 5-7 days.

Take the lesson learned from this experience and cherish each day with your wife and child!  I am sure after such a scare you won't make the same mistake!  I am soon to be married but made a mistake and could potentially have lost the love of my life.  Again, I'm glad to hear you turned out alright!!  By the way, what country are you in?

HEY joe1888 that great news. I am sure that you are fine now. You have a 10% chance of the next test being positive and more than likely that won't happen. I also have just got the girl I had unpropectes sex with test and she come out negative which this would be her conclusive test since it's been 13 weeks since her past exposure before me. It's been 8 weeks since we had sex and I had ALL kinds of symptoms, night sweats, swollen nodes, rushes, you name it, I had it and the whole time she never had the disease. I'm sure your symptoms are all stress/anxiety related, just like mine were and I'll turn out to be fine.

I've been following up on your story, and I'm happy for you that your test was negative up to now! I'm sure you'll be fine!!

I'm a nervous wreck about my situation as well, I'm only at the 3 and a half week mark, so far had 2 tests that also came back negative! I'm praying for everyone on here!

Just wanted to share:

Took a 6 weeks test today at a Singapore Government approved Anonymous HIV testing center. It was a rapid HIV test that was being performed by the clinic's doctor.
Thankfully the result is still negative at 6 weeks.
The Doctor told me that 6 weeks is usually accurate up to 90% of the time.

Additionally, I also opted to take an anonymous DNA PCR there.
The results will be available in two weeks time.
And I really really really hope that it will be negative.

What was being diagnosed visually be the local doctor to be 100% sure of genital herpes back home, was being refuted by one of the dermatologist here in Singapore.
The Singapore dermatologist told me that by visual examination, she's 100% sure that the small vessicles that I had on my genital foreskin was not genital herpes and that it was fordyce spots. Now I am even more confused. I am going to see an infectious disease specialist tomorrow, she had her post-doctoral at Harvard Medical School and also practiced there during here post-doctoral terms there.

I feel puzzled because there's no other explanation for the vessicles to be genital herpes and last this long (6 weeks), and had no effect whatsoever even after taking a two weeks dose of acyclovir. The vesicles are very small, never grow to become larger one, never crusted, and never form any kind of ulceration throughout this 6 weeks. The old vesicles and some new vesicles that are forming never caused me any itch, irritation, or pain.

My condition overall still haven't improved. Just had another diarrhea. Just had a headache and just recently felt rather hot and sweat my shirt. Fortunately no fever, no sore throat. Yesterday, after my median rhomboid glossilitis is gone, I even had a black and brown tongue where I had successfully cleaned it using a toothbrush. But at least for tonight, my tongue looked fine.

@struggler17: you have had a 12 weeks negative test where every expert on this planet will conclude as being conclusive. I guess whatever punishment that we are in, is not necessarily being defined by CDC. Don't you agree?

Hi all, I am bit terrified that My tingling sensation in legs and arms dont go away. I had sex with a prostitute and condom broken off.
1)After 1 month of exposure,elisa test 1 and 2 :result -ve
2)At 85 days ie 27th Jan2009, after exposure ,I tested -ve for P24 antigent test hiv1 &2.

During 1 month of exposure I had night sweating and sleeplessness.

What tests should i take so as to get conclusive test.My abdoment is still upset.I  still have  unhealthy  tongue.I am going to be married.So i need to get tested completely.Please advice. I am in india.
