HIV Infection

When i was in Prague I Visited a Night Club on February 19th and had Protedted Sex with a Prostitute. After some time she took out the existing Condom and replaced it with a new one. We continued the Vaginal Sex and suddenly when she checked the condon it had slipped into the Vagina and the penis was exposed . I did not ejaculate inside her. She pulled out the Condom from inside here Vagina. I immediately pulled out and washed with soap and moved from that place. I checked with the Night club about health testing and they said that all the girls are medically checked. But after two weeks i got some yeast infection in the penis ,i applied Clotrizamole on the same , it cleared off in three days. Same time i also got a Lymph node under the armpit enlarged for a day. I also got a Sore throart for two days. After 5 Weeks i went to a Dermatologist and checked for all the STD including HIV . They did a swab test and did a blood test for Syphillis and HIV . All the report came out negative . The Blood test report was as follows

HIV COMBI                : Negative
RPR IMMUTREP             : Negative
Syphillis recomb , ELISA : Negative

The detmatoligist anyway told me to get it checked after three month . I really do not not if i am infected. But by reading your previous replies i get some satisfaction on the Women to Male transmission and also the transmission through a single episode of the Vaginal Sex.

Doc, i wanted to know what is HIV Combi test ( is it the Antigen + antibody test) . If i was infected this result should have shown Positive by five weeks if my assumption is correct about the test. What does the other tests mean .

I am really worried about HIV infecction. If you could clear my questions it will be really helpful. what are my chances of HIV Infection.

Let's assume there is a 10% chance your commercial partner had HIV.  (She probably didn't given the history of regular testing.)  Now assume 1 chance in 1000 of transmission to you if she was infected.  Now assume the testing you had at 5 weeks would have picked up 99% of infections, i.e. there is a 1% chance that if you were infected and the test missed it.  To calculate your odds of actually haveing HIV, those odds are multiplied.  Thus 0.1 x 0.001 x 0.001 = 0.0000001.  That's one chance in 10 million.  Perhaps you will agree with me those are pretty good odds in your favor!

You've got it right about the combination test:  it checks for both the virus itself (the antigen part) and the body's reaction to the virus (antibody).  At 5 weeks, normally one or the other will be positive, often both.  Some experts would advise being retested at 3 months, as your dermatologist told you.  But maybe you'll be comfortable with the current low risk of 1 in 10 million.  But if you're still a little nervous, get retested at 3 months.  In the meantime, rest easy.  You're fine.

Good luck--   HHH, MD
