Let's assume there is a 10% chance your commercial partner had HIV. (She probably didn't given the history of regular testing.) Now assume 1 chance in 1000 of transmission to you if she was infected. Now assume the testing you had at 5 weeks would have picked up 99% of infections, i.e. there is a 1% chance that if you were infected and the test missed it. To calculate your odds of actually haveing HIV, those odds are multiplied. Thus 0.1 x 0.001 x 0.001 = 0.0000001. That's one chance in 10 million. Perhaps you will agree with me those are pretty good odds in your favor!
You've got it right about the combination test: it checks for both the virus itself (the antigen part) and the body's reaction to the virus (antibody). At 5 weeks, normally one or the other will be positive, often both. Some experts would advise being retested at 3 months, as your dermatologist told you. But maybe you'll be comfortable with the current low risk of 1 in 10 million. But if you're still a little nervous, get retested at 3 months. In the meantime, rest easy. You're fine.
