Hemhorrhoids and anal-finger play with HIV positive partner

Dear Dr Hunter,

Many thanks in advance for your advice. I have recently started dating an HIV positive woman who is very well but not yet on any treatment. We haven't yet had intercourse and have limited ourselves to mutual masturbation. Two days ago while we were intimate as well as masturbating me she also put a finger in my anus for a few minutes.

I have a history of piles and while they were not playing up at the time today I noticed some blood on wiping. Her fingers don't usually have any cuts on them and we don't think she had any when we were being intimate, but we didn't properly look. Today she noticed a slight graze on the finger she thinks she played with me. I've read the other posts on the site but can't find one that particularly addresses this. Just in case it was there when we were playing with each other is there a siginficant risk of HIV transmission? And if so, do you think I should see a physician to start on PEP as I know the window period is approaching.

Many thanks for your help: just to clarify it was my blood from my hemmorrhoids, and potentially a wound on her finger.

Could I just confirm that even if her wound was oozing clear fluid at the time while my hemmhorroids were potentially bleeding there would be no risk (though we're uncertain if that happened or not). No follow-up questions, I promise, as I sincerely appreciate your valuable time.

As I understand the situation you are describing, the blood you describe came from your hemmorrhoids.  If that is the case, there is no risk to you and certainly no need to even consider PEP.  It is her blood that is potentially infectious, not yours.  Whether or not she had a graze or other shallow wound on her finger does not change the situation. Still no risk.  Hope this helps.  EWH
