I honestly do not believe that you have put yourself at any significant risk of an STD because a condom was used for both vaginal and oral sex, and you seem sure about the fact that the condom remained intact and in place. You seem to be unsure about the possibility of some vaginal fluid gaining access inside of the condom. Obviously if this is the case, there is a possibility of contracting an STD, more likely conditions that are common and very easy to transmit, such as Chlamydia and Gonorrhoea, less likely for blood-borne viral infections, as you would require larger amounts of infected vaginal fluid. However if you are in doubt, my advice to you is to have a full STD screening, including HIV (a HIV Duo test would be conclusive after 4 weeks of the incident) and obviously not to have intercourse with your wife until you know that all the tests are clear. As mentioned above, I would estimate a very low or even zero risk of HIV infection from this sexual encounter, as described by you.

There is no debate (among experts) about the HIV risks associated with oral sex. The risk is so low that almost nobody who cares for HIV infected patients has ever had a patient believed to have been infected that way. Among experts, it's a semantic issue about using terms like "no risk" and "very low risk". There is no difference between my or Dr. Hook's use of "low risk" and other experts' "no risk".

Another question Dr. Jose,
I'm in Hong Kong and there is only one place to have a HIV duo test, as I believe. they call it P24 antigen and antibody test? But nobody recommend this and also nobody comment on this. If P24 antigen is available with antibody in the test. is that means it is HIV duo and will it be conclusive at 4 weeks?

I asked the same thing from Dr. Hunter as I desperately needed an answer. His answer was I'am not at risk and it is very natural to exchange some fluids even for mutual masturbation. Also he stated that hand to genital transfer never happen. Also he said that I don't need a test as I'm not at a risk for HIV but only for peace of mind I can do it.
But with your comment, I'm little confused. Can hand to genital transfer take place?
