I need closure on my risks and symptoms!

Hi Dr,

I had 18 counts of protected vaginal and unprotected oral sex with CSW this year, with the last exposure on 13 November.

1) Am I risk of HIV from the protected sex and unprotected oral sex?

2) I am also concerned of the risk of fluid transmission when the condom was removed on several occasion and oral sex or masturbation was performed subsequently. (e.g fluids from uncovered part of the shaft or touching the condom)

3) In the last encounter, oil based lube was used and there was penetration for a few minutes. The condom did not fail. The condom was replaced on my request and proper lubrication was used. Would the use of the oil based lube present a risk even if the condom did not break?

Did a STI test a week back and waiting for results. It was upon the realisation of the potential risk of HIV when I decided to test did I start to notice some weird symptoms.

1) Fungal rash on armpits confirmed by dermatologist 2 weeks after exposure. Cleared up with micanozale cream. Didn't think much of it at the time as HIV/STI fear was not on my mind, accredited it to re-wearing of clothes without washing.

2) Stiffness of the neck and what seems to be a cerviconegic headache + disorientation. I have a lumbar disc herniation but have not checked out my cervical spine. 10 days ago till now. Came after I spent a night or two staying up research symptoms on the computer.

3) Warm sensation especially around the back of neck. But no fever and highest temperature was 36.8. A live-in family member has been down with fever and sore throat about a week ago but I don't recall alot of contact.

As you can see I am trying to convince myself that my symptoms are of other origins. Do you see any red flags as to ARS?

I am intending to do a combo test tomorrow (almost 6 weeks post exposure) if the clinic is open. I could really use some comforting advise.

Welcome to our Forum.  I'll be happy to address your questions.  the short version of my response, before I address your specific questions is that there is no risk for HIV from the activities that you describe- there is no evidence that HIV is transmitted  through receipt of oral sex and condom protected sex is safe sex.  As for STIs, you were screened and will soon have test results, but the sex acts you describe have very little risk for other, non-HIV STIs either.  I presume that your tests will be negative.

With that, let's work through your questions:

See above- no.

2) I am also concerned of the risk of fluid transmission when the condom was removed on several occasion and oral sex or masturbation was performed subsequently. (e.g fluids from uncovered part of the shaft or touching the condom)
Again, no risk.  HIV begins to die as soon as it is outside of the body and as a result neither transfer of genital secretions on a person’s hands while taking off condoms or during masturbation represent any meaningful risk for HIV or for other STIs.

3) In the last encounter, oil based lube was used and there was penetration for a few minutes. The condom did not fail. The condom was replaced on my request and proper lubrication was used. Would the use of the oil based lube present a risk even if the condom did not break?
The problem with oil based lubricants is that they weaken condoms increasing the likelihood of breakage.  If they do not break, then you are OK.

As for the seond group of questions:

This is not a sign of HIV, recently acquired or otherwise.

See above- not a concerning finding for HIV either.

Same answer, not a concern.

Sorry for the double post, but is the test I did considered 4th generation?

I did a determine HIV antigen/antibody test on the 26/12, which puts me at 5.5 weeks post exposure.

It was non-reactive. Can I move on with this as a conclusive result?

Yes, any and all approved antigen/antibody combo tests will provide conclusive results about HIV infection at any time more than 4 weeks after exposure. EWH

Merry Christmas Dr Hook. Appreciate your response. I am hoping tomorrow's testing will finally bring me closure to this past few weeks of stress and anxiety.

My parting question will be: Will a combo anti-body test at 6 weeks exposure be conclusive in your opinion?
