Am i at risk - please help

Hi doctor,

I had been to asia 7 weeks ago and had protected  sex for 3mins and unprotected oral (receiving) for about 2 mins from a woman escort. The next day i had a burning sensation on my buttock and gotten bad after a week. So i went for a HIV test and  STD screening after two weeks and all came negative. The third week i had  herpes1 and herpes 2 test  and came postive for herpes 1 and  2 was negative. I still had the burning sensation and it went away after four weeks but caught cold,and dry cough with blood smell when i cough and seems to lingering around for almost two weeks now but no fever .Had sore throat for few days and went away.
I went for an oral other test after four weeks of exposure, the result came  negative and tested again (oral rapid test) after 48 days of exposure, came negative. At times i still have the burning sensation on my buttock and around my anaus and dry cough with bloody smell. My questions are as follows,
1 . Am i risk of getting HIV  or other STD's?
2. How accurate theoral test is after 48 days of exposure? This is costing my marriage and me and my wife never used condom and  never had any other partner in my life. Can i put this behind and move on with my regualr life?
3. what could be causing my butt to burn? Do you recommed seeing a urologist?
4. what is the possiblity of getting HIV thru Oral and herpes 1 is i had gotten from her?

This is full of my mind and can't concentrate on anything due to the and not able to be close with my wife and not able to carry on my normal
activity. Please advise. thanks a lot

I said above that you did not need further testing and see no need to change my recommendation.  I do however feel the need to stop fueling your anxiety.  Therefore this post is over.  You do not have HIV!  No further questions please - they will be deleted.  EWH

Like many others your guilt is manifesting as HIV anxiety. You're not looking at the situation rationally. At most, you had a theoretical risk, which means HIV is not contacted at any measurable frequency from the exposures you describe.

The doctor already told you that for your 'exposures', your 48 day test was conclusive, and you don't need additional testing. You paid $15 to get the opinion of an HIV/STD prevention specialist- BELIEVE IT and MOVE ON. Your GP doesn't have his expertise when it comes to HIV testing.

Thank you very much for your explanation. Just to stop fueling my anxiety i went for an other blood work for HIV and STD test after 54 days of the exposure. All came back negative but the nurse and the doctors insist on testing after three months and six months. One last question, the result after 54 days is conclusive? Can i put this behind and move on with my family and have unprotected with my girl partner. Do i need an other test after three months? Please answer my last question

You are worried but i am not.  please re-read my comment.  you are not at risk.  I will comment briefly on some of your persisting concerns.

1.  Read what I said about your test results.  This indicates that you are not infected  If you didn't like the oral test or don't trust the way the specimen was collected, get one performed on your blood but it will give you the same result.
2.  Your symptoms are meaningless.  Look at other posts on this site and you will see that we say this repeatedly.
3.  As for your guilt, there is little I can do for this other than to tell you to figure out for yourself how to deal with it.
4.  finally, the reason people often use the 12 week recommendation for final testing is to be most conservative and careful, without considering the person's actual situation.  My recommendation is based on the fact you have given me.  EWH

Thank you very much for your reply. But i am still worried for the following reasons. I have swallon glands on left side of my throat and had severe tongue soar for a week. The cold and the dry cough is still lingering around for the past two weeks and i have seen my family doctor and was put on antibiotics but i didn't tell my doctor about the exposure i had. Here are my questions, The oral test i had after 48  days of exposure is it conclusive,?they insist on coming back after three months of exposure to be conclusive test. The nurse did a quick swab on my teeth gum to do the oral test. Is that quick swab has the good sample for the test. The nurse said that the test line shows that the test is working but i am concerned that she might not have gotten a good sample for the test in the quick swipe on the front gum teeth

I am so afraid and the sense of guilt that i might be putting my wife at risk. should i use protective (sex) means till i get tested after three months but so far i had unprotected with my wife. the doctors on the health department say that the 7 weeks test is not conclusive and it only shows that i am negative three months before.Do you recommend going for an other test after 8 weeks? My sore tongue, sore mouth and a prolonged cold with my swallong glands indicate anything of HIV related? I am so worried and can't concentrate on anything. Please help

Your exposure was very low risk.  Condoms work and receipt of oral sex is rarely associated with HIV acquisition.  You ave proven that with a negative HIV test at more than 6 weeks;  at 6 weeks over 95% of the persons who are going to acquire HIV have positive blood tests.  When considered in context with the low risk nature of your exposure, you can be confident that you did not get HIV and move forward with your life.  As for your specific questions:

1.  NO.
2.  See above.  If you had gotten HIV odds are your test would be positive at this time.
3.  No idea.  I am confident that it is not related to the exposure you describe.  My advice would be to see your regular health care provider and work through things with him/her.
4.  HIV- less than 1 in 10,000 if your partner was infected which in and of itself is unlikely.  Herpes, at this time is likewise improbable.  Herpes lesions typically appear in less than a week after exposure.

My advice.  Put this behind you and move on.  No need for further testing.  EWH
