Help

Last sunday 22 Jan, An escort came to my hotel room in Times square, Manhattan for (385$). At first she offered a condom and we did protected blowjob and sex but after that I asked her to continue without the condom. she refused but I convinced her with extra money (300$) she agreed.  We had ( Unprotected ) blowjob and sex for like 2 minutes. I ejaculated a little in her while my penis is coming out. I couldn't sleep. I'm afraid she has HIV. She said she teaches pre-school and she looked healthy. Now I have red dots on my thigh. is that bad news?

This is a comfusing result; HerpeSelec results normally are given as an ELISA ratio; you would expect to see a number followed by two decimal places, like 0.72, 1.46, 4.29, etc. I'm unaware of any type-specific HSV antibody test that reports results in u/ml.  However, new assays are becoming available all the time, so this doesn't mean the result is unreliable.

The rest of this reply assumes the result is valid.

1) You have been infected much longer than 9 days -- 3 weeks minimum and probably 3+ months.  Beyond that, it's impossible to say.  But it seems a good bet you were infected by some distant past partner, before you were married.  Or that your wife is infected from a past partner and you caught it from her.  Of course I cannot judge your and her sexual lifestyles, risks, etcl; that part is up to you.

2) Again assuming the result is reliable, you are infected with HSV-2 and can transmit it to your wife, unless she already is infected (either before your marriage or from you).  Your child is not at risk; sex is the only possible mechansim of transmission.

3) Here is what to do.  First, tell me the details of the test; find out from your doctor or the lab exactly what test was done.  Also, if an HSV-1 test also was done, tell me that result.  Second, assuming your result is valid, the next step is for your wife to be tested for HSV-2, to learn whether she is already infected or not, in which case she is susceptible to infection from you.

I may have more advice after learning more about your test.

The gonorrhea and chlamydia results are reliable.  The HIV results are not; done too soon.  Feel free to post final test results at 6 weeks or later; until then, no further comments please.

I did gonorrhea/chlamydia after 8 days of exposure
all came negative
and I did also the HIV test after 8 days. I know its not accurate and I will do it after 6 weeks and 12 weeks.
the results all negative.

Any doctor can order these tests.  A urologist of course can do it, but there is no need to see such a specialist just for this test.

I already answered about HIV risk and testing.  You are at sufficiently low risk that in my opinion you can continue unprotected sex with your wife.  But if you want absolute proof, you'll have to wait 6 weeks, after the HIV test.

It was my first time and definitely the last. I'm praying it won't be the mistake that will destroy my life.

I planning to do the gonorrhea/chlamydia and HIV tests but can Urology doctor help me out on these tests.
you said it is safe to resume unprotected sex with my regular partner if the results of gonorrhea/chlamydia are negative. does that eliminate the infection of HIV.

Welcome to the forum.  I'll try to help.

You are overreacting to a very low risk event with respect to HIV, and pretty low as well for other STD.  Let's start with a comment you wrote on one of the community forums: "if she has HIV (GOD FORBID) is it 100% i'll get it".  That shows a serious misunderstanding.  When a woman has HIV, the average HIV transmission risk from unprotected vaginal sex is once for every 2,000 events.  That's equivalent to having unprotected sex with infected women for 5 years before transmission would be statistically likely.  I'm not belittling the risk, and of course that's why condoms are important.  But even if your partner had HIV, transmission was by no means inevitable.  (Many spouses of HIV infected people never catch it, despite years of unprotected sex.)

And it is very unlikely she had HIV anyway.  The large majority of female sex workers do not (under 1%), and expensive escorts probably are especially unlikely to be infected.  Yours might be at slightly higher risk; if she made an exception about condom use with you, it's likely she does with other clients.  But still the odds here also are very heavily in your favor.  (Still, it was really, really dumb of you to insist -- and to tempt her with such a large amount of money.)

There is no STD, and certainly not STD, that causes "red dots" on the thigh.  My guess is that this is entirely unrelated to the sexual exposure.  Maybe folliculitis, or maybe nothing at all.  Anxiety after regretted sexual exposure often makes people notice genital area skin anomalies that are not new or abnormal.

From a strict risk assessment perspective, you don't need HIV testing; the chance of exposure and/or infection is too low.  However, this is a circumstance that does warrant testing for other STDs.  I recommend you visit a doctor or clinic (e.g. your local health department STD clinic) in the next day or two for a urine test for gonorrhea and chlamydia, and for examination of the spots on your thigh.  Then plan on a syphilis blood test in about 6 weeks.  When you do that, you may as well have an HIV test as well, but not because I really believe you are at significant risk.

In the meantime, if you have a regular partner, you can safely resume unprotected sex once you have the negative gonorrhea/chlamydia results, with no fear of infecting her with HIV or syphilis.  If I were in your situation, that's what I would do with my respect to my wife.
