What could this be?

I presented to the ER with pressure in the chest and general weakness.  They immediately began a full cardiac work up.  The ECG, chest x-ray, heart ultrasound and subsequent stress test were unremarkable, but the blood work showed cTnT 0.20 ug/L, Myoglobin 440 mg/L, and CK 409 U/l.  I am a 30 year old female in great health with no other health issues a part from benign symptomatic PVCs and ventricular couplets.  Considering my age what are your impressions based on this information? What could cause these elevated values on my blood work?

Is that level of TNT elevated in your laboratory? If so and despite the normal stress test I would ask for a left heart catheterization, with ergonovine to test for prinzemetal's angina. This will be more likely in your age group than typical coronary atherosclerosis with plaque rupture and is treated differently. Another possibility is that you had takasubo's myocarditis, although if you had a normal echocardiogram this is virtually excluded.
