Increasing Lotrel strength

Hello, I've been taking Lotrel 5/20 for the past 5 years to control my hypertension.  However, based on my latest blood pressure readings, I think I may need a stronger dose.  My question is...how does the doctor determine   which would be more effective for me:  increasing my dose to 5/40 or increasing it to 10/20?  Is there a rule of thumb to determine which of the meds in Lotrel should be increased?  I thank you for your assistance.

It depends mainly on the results of your laboratory studies, as one of the components can affect your kidney function. Also the other component can cause lower extremity edema, and so its dose may be limiting. his the problem with combination medications, as they cannot be independently titrated.
