Thanks for posting this.

What do you think of putting it in the Health Pages? It would be great to have it handy for the many, many times the subject of disclosure comes up.

I have nothing to disclose anymore and grateful for that.  However, in light of recent discussion, read through this with great interest.  There are some suggestions in here that I would have made use of if I had to do this again.  Great info and thanks for posting this.
