Exposed and Hating Myself

Dear Expert-Thank You for this service.

I am a 30 year old male. On December 1st 2011 I had unprotected vaginal sex with a girl whose past I am not well aware of. We also performed unprotected  oral sex on each other. We had sex twice this night. On December 2nd, the same thing happened again. On both nights, I was under the influence of Alcohol, which explains a lot. The room was dark.

On the third day, I started feeling a burning sensation around the crown of my penis, as well as the tip (not sure of the terminology, Sulcus perhaps?) and it would not go away. I also felt stomach cramps. I called her, and asked her if she ever had been tested, and other questions of the like. She told me that she gets 2-4 yeast infections annually and that nothing other than that ever happened, nor did she get tested, ever. She said her "Pap Smear" was okay as well.

I urged her to consider getting tested. She was nice enough to accept my request and got tested for a bunch of STDs on December 7th 2011.

The following are her results for Herpes IgM and IgG:

Herpes 1 & 2 IgM
Result is 20.05 NTU (NTU is the unit, I am currently in Pakistan)

The range mentioned was 11 NTU is Positive
Between 9 and 11 is "Grey Zone"

Herpes IgG 1 & 2
Results "Reactive" S/CO (signal/cut off) Ratio 11.3

Range

Non-reactive if S/CO is =1.1

Indeterminate if it is between 0.9 and 1.1

She was, and has been infected, and I have been exposed.

I am looking for a response, in which the reality will be laid out for me of how things will be for me from no onward. Also, how I should go about testing in the near future. I am not feeling other symptoms at this moment. I had a mild sore throat but it seems to come and go. I don't think I am running a fever. There seems to be no discharge. I do realize it's probably early for these to surface. I definitely feel burning around the head of the penis.

I have contracted it right?

I have herpes and I have been with my partner for a year and he hasn't gotten it yet. If you do happen to have it it isn't the end of the world. There are lots of people who have it and do not know they are infected.

You certainly may not have gotten it.  You'll have to sit tight and wait, I'm afraid.

There is an excellent chance that if she has it, you don't have it!  Transmission doesn't happy with every intercourse, just because someone is infected.  Unlike gonorrhea or chlamydia, which is far more likely to be transmitted.

She is going for the type specific test in two days. She also saw her gynecologist today who ordered the specific tests and examined her to conclude that though she may be in a Herpes outbreak right now, she does not have lesions or sores.

Are my chances of having contracted it substantial, or is there any statistic suggesting there's a slight change I may not have gotten it?

Thank You Terri.

I'm a little surprised if you've had a cold sore that your HSV 1 test is negative, but that does happen, the test isn't all that sensitive for HSV 1.

I would recommend retesting 4 months after the encounter and see what you get.  I would also suggest that she get type specific antibody testing vs. what she has.

P.S. My last STD panel test was done on October 21st this year. Everything was negative, and I haven't had any recent exposure/sexual contact of any sort before October 21st or after October 21st until this December 1st incident.

Yes, I believe I have in fact had a cold sore in my lip area. A couple of times I was shaving and it got sliced off, which was surprisingly the end of it. Though it hasn't been a problem in my life, it has happened a few times. I don't know if it has anything to do with the fact that I had a bad case of Chicken Pox when I was younger.

I've lived in the US mostly and on average I've been tested twice a year, for all the major STDs, including HSV 1 and 2, the results have always been negative.

Thank You for your help.

OK, so here's the deal.  The IgM is useless.  There are many false positives, and people who are already infected regenerate IgM so we can't make anything of this.  The provider did the wrong test  - they should have done separate HSV  1 and HSV 2 tests, but apparently, they did a combined test, HSV 1 and 2, which tells us only that she has been infected with one or the other, or there is a false positive on this combined test, which is common.  In other words, we can't make sense of this test result.
Also, if she is infected, that doesn't mean that you are.
have you ever in your life had a cold sore on your lip?
Don't jump to any conclusions here!
Have you been tested yourself?
