Internal Carotid Artery Dissection

I'm a 32 year old male who was diagnosed with a spontaneous dissected internal carotid artery 6 weeks ago.  This resulted in several TIA's and a week in the hospital.  Treatment consisted of heparin followed by warfarin.   I'm curious what symptoms would indicate a need to return to the hospital.

I have the following symptoms:
   1) My neck hurts on the side of the CAD (Left).
   2) For the last 3 weeks, my neck also hurts on the opposite side of the ICAD (right).  The pain on the right side is very similar to the pain on the ICAD side in that hurts both in the side and the back of the neck.  
   3) I sometimes have a feeling as if someone is choking me - there seems to be pressure across the front of my neck but I can still breathe.  
   4) I can sometimes hear and feel what sounds like tiny bubbles ascending the back of my neck.  They usually come in a short burst while I'm sitting on the couch.
   5) I commonly have short periods of dizziness where I have trouble focusing my eyes.

Would any of the symptoms above indicate a need to return to the emergency room?  What symptom would require a trip to the hospital?

Many thanks for posting the question via my forum. Before answering, I would like to remind you that , my post is like a guide for you. But to come to a pinpoint diagnosis, I will repeatedly as you to visit the treating doctor or meet a good consultant.
Any condition that is decreasing your efficiency, making your efficiency low, demands you to have a re-talk with your treating doctor.
However you would  be required to go back to the Emergency room if you experience -
1) Severe Headache,
2) Persistent Nausea,
3) Throbbing pain,
4) Bleeding from Ear/Nose,
5) Weakness of some body part.
