lumbar pseudomeningocele

My first spine surgery was a decompressive disc surgery L3, L4, L5. I've had a dura tear resulting in a Dx pseudomeningocele (CSF) ..   The leak recured and a second surgery to repair the leak was made.  After two weeks from the operation the leak had to be repaired again. It has now been 9 weeks and the extreme unbearable headaches are back. MRI was taken today;do not have the results. Dr's diagnosis is lumbar pseudomeningocele. No other info given by him at this time.
What would cause the recurrence?  What are the different ways that this can be treated besides just CSF?  A dura repair surgery ???   Dangers ???

a bllod patch was used on the surgery. I failed to mention that.
Currently the Dr. has prescribed Butalbital/apap/caff .. they help some; better when taken with caffine drinks.  The Dr is renowned for this area, however he did say perhaps to get a second opinion. Suggestions for another dr.

Thanks for writing in.
It is difficult to tell as to what caused the recurrence. The initial management entails closed external drainage of CSF with or without blood patch. Failure of non operative measures may necessitate surgery. The procedure involves repairing the dural defect and repairing the encapsulated cavity of pseudomeningocoele.
