I am a neurosurgeon from Chicago.  It sounds like you have a disk herniation.  Part of this has some inferior extension where it is in a good position to trap the descending nerve root against the undersurface of the facet joint and cause pain.  I could tell much better by viewing the film but it sounds large and particularly if you have any leg symptoms, numbness on your bottom or difficulty with your bowell or bladder you should seek medical attention.
