Has my 6 yr old got a medical problem or is it me?

Hello i'm a bit new to this but at my wits end.   My 6 yr requires guidance in everything. He daydreams and I have to issue him with things to do one by one or he forgets.  His moods swing, he is either totally lovely (not that often) or he is really really naughty, moody,answers back, it doesn't matter he almost knows he is going to cause a situation but still does it.  He has seen me cry at him, shout at him and he does not seem to care about my feelings one little bit.  He only cares if you take something away that he likes (like playing with his friends) but even that does not deter his moodiness.  At school they don't seem to have a problem, which is my problem as he behaves, so everybody presumes it must be me.  He does require additional help with learning mainly due to his concentration span.  He seems obsessed by death and people dying at the moment and I try my best to explain.  He is lying, he always talks over people, and the most common problem with him is as soon as you say no he starts punching the air his bottom lip drops and then he sulks and start saying comments that will hurt people. He has already told me that he wants to live with his nan and i'm so so hurt.  He was such a sunny lovely baby and I feel like i've lost my little boy.  Can anybody help or been through similar problems?

Maybe back off a bit and not concern about his moods, let him be, if hes not hurting anyone or throwing big Tantrums , kind of let him get on with it,a lot of kids answer back it showing they are getting some independance .As there is no problem at school it is possible its from Home. Take it easy and see what happens, has he a Dad around and Can he do more Guy things with him or a Granpa .
