It also has a LOT to do with the employer, if you are covered through your Group Health Plan. Employers dictate what they want covered, etc since they usually pay half of the premiums.
When I was having fertility issues I had Empire, it was a government plan (I was a school counselor). They covered a certain amount--- everdid haveto use it!
I think it depends on the state also. Some states now mandate it while others don't.
Humana does have fairly decent coverage. I think most of the bigger companies probably do but it depends on the actual plan. You know they have so many choices. We chose the higher plan thru Humana for that reason.
