I'm glad it has helped... when my Dr. told me to submit a birth plan, I was like... "What in the world is that?"  I (as always) researched it and that was the most extensive that I could find.  I wish I could've posted the actual URL, b/c it even talks about a few more things that really weren't important to me but I think this is a good start for anyone else though... anyways...

Ladies, we are closer and closer... I am in my 32nd week now and I just keep thinking... 37 weeks is considered full-term... am I ready to have a baby in as little as 5 WEEKS!!!  I am ready to meet him and to be a mommy, but the labor.... wow... I have to admit I'm a little nervous!!

this is a great plan.... i wish mine could be that excessive, but since i am having my 4th c-section...alot of that is not available for me. such as the breastfeeding within minutes of birth... i will have to wait until i go to recovery(about 15min after birth)

good luck and as long as all goes well, i am sure that they will be able to follow your plan.

Oh my gosh all of those decisions!  Makes me nervous!!  I thought you just go in there and get it done!  Now I'll have to think about all of this and talk it over with my doctor!

Okay, I know what is happening... I cannot post a URL on this thing... so, tarrah, I will just copy and paste mine here and you can modify it... hope it helps:

Name
Due Date
Dr.
Hospital

As a first time mother-to-be, I understand that there are many unknown variables in childbirth that could alter my "birth plan" at the last minute. However, as long as things are going well, and neither my life nor my baby's life are in jeopardy, I would like to stick to the birth plan that follows.

________________________________________
LABOR
•  I would prefer to avoid an enema, being catheterized and/or shaving of pubic hair.

•  I would like to be free to walk around and/or change position at will during labor.

•  I would like to be able to have fluids by mouth throughout the first stage of labor.

•  I would prefer to keep the number of vaginal exams to a minimum.

•  I do not want an IV unless I become dehydrated.

•  I do not wish to have continuous fetal monitoring including internal monitoring unless it is required by the condition of the baby.

•  I would like to wear contact lenses or glasses at all times when conscious.
• I would like to be able to take a warm shower during labor, if I decide to do so.
• I would like my husband to be able to monitor the number of visitors coming into the room.
• I would prefer that no students, interns, residents or non-essential personnel be present during my labor or the birth.
________________________________________
LABOR AUGMENTATION/INDUCTION
•  I do not wish to have the amniotic membrane ruptured artificially unless absolutely necessary.

•  I would prefer to be allowed to try changing position and other natural methods before pitocin or any other intervention is administered.

________________________________________
ANESTHESIA/PAIN MEDICATION

•  I realize that many pain medications exist I'll ask for them if I need them.

________________________________________
CESAREAN

•  Unless absolutely necessary, I would like to avoid a Cesarean.

•  If a Cesarean delivery is necessary, I would like to be fully informed and able to participate in the decision-making process.

•  I would like Charles present at all times if the baby requires a Cesarean delivery.

•  If the baby is not in distress, he should be given to Charles immediately after birth.

________________________________________
EPISIOTOMY

•  I do not want to have an episiotomy.

•  I would appreciate guidance in when to push and when to stop pushing so the perineum can stretch.

•  If possible, I would like to use perineal massage to help avoid the need for an episiotomy.

________________________________________
DELIVERY

• I would like only my husband, Charles, and my mother, Anna, to be present during the delivery.

•  I would like Charles and/or nurses to support me and my legs as necessary during the pushing stage.

•  Even if I am fully dilated, and assuming the baby is not in distress, I would like to try to wait until I feel the urge to push before beginning the pushing phase.

•  I would like to have the baby placed on my stomach/chest immediately after delivery.
• I would like still photos/videography to be taken by Charles and/or my mother as the doctor places the baby on my stomach/chest immediately after delivery.
________________________________________
IMMEDIATELY AFTER DELIVERY

•  I would like to have Charles cut the cord.

•  I would like Charles or myself to hold the baby while I deliver the placenta and any tissue repairs are made.

•  I plan to keep the baby near me following birth and would appreciate if the evaluation of the baby can be done with the baby on my abdomen, with both of us covered by a warm blanket, unless there is an unusual situation.

•  If the baby must be taken from me to receive medical treatment, Charles or my mother, Anna, will accompany the baby at all times.

•  I would prefer to hold the baby rather than have him placed under heat lamps.

•  I do not want a routine injection of pitocin after the delivery to aid in expelling the placenta.

•  I would like to delay the eye medication for the baby until an hour or two after birth.
•  After the birth, I would prefer to be given time to urinate on my own and not be catheterized.
• I would like assistance in breastfeeding immediately following the delivery.
________________________________________
BREASTFEEDING

•  I plan to breastfeed the baby and would like to begin nursing very shortly after birth.

•  Unless medically necessary, I do not wish to have any bottles given to the baby (including glucose
water or plain water).

•  I would like more information about breastfeeding.

•  I would like to meet with a Lactation Consultant.
________________________________________
POSTPARTUM

•  I would like a private room, if available.

•  Unless required for health reasons, I do not wish to be separated from my baby.

•  I would like to have the baby "room in" and be with me at all times.

________________________________________
CIRCUMCISION

•  I would like the baby to be circumcised before we check out of the hospital.
• I would like guidance on caring for the circumcision site before we check out of the hospital.
