The first trimester is weeks 1-13. The second trimester is weeks 14-26. The third trimester is weeks 27-40 (it has the "bonus" week because 40 isn't evenly divisible by 3). So the day you reach 26 weeks is the last day of the second trimester, and 26w1d is the beginning of the third trimester because it's the beginning of the 27th week.
