Sorry to hear about your suffering. I spent many years in exactly this type of pain. Over time, I began having terrible sinus issues. I finally got help from a teacher who taught that relaxation practice to release tension would solve most if not all pain issues. I personally found that if I did not stop all stimulants (coffee was the culprit with me) then I could not release the tension and stayed bound up in pain. I have fibromyalsia, so my health care provider was shocked when I reported that I had decreased my pain level from a 10+, to a 1 to 2 most days, just by stopping coffee and practicing relaxation, while sitting in quiet stillness for 20 minutes everyday.
The drugs you mention are very drying, and I found that to be a particular issue. I stopped all meds and had much faster improvement. It is very important to sip water all day to stay hydrated and to eat plenty of fruits and vegetables everyday. Might sound too simple, but how many people do you know who have fibromyalsia, and manage their pain without medication. Yeah me! The shoulder-neck-back pain could be secondary to arthritis/bursitis of the shoulder, arthritis or degenerative disc disease of the cervical (neck) spine or to compression of one or more nerves after they have emerged from the bony spine. You will need a good physical examination of your shoulder and neck and then have X-rays of your spine, to pin-down the source of the pain. The headaches, in this situation, are probably secondary to muscle spasm around the painful areas in the neck. Your best bet would be to see a rheumatologist (arthritis doctor). It would be a mistake to have massage or manipulation therapy before the actual cause is diagnosed.
