From the X-ray report you have provided that describes multiple tiny nodular densities, the suspicion that these represent “granulomas” is probably correct. This is the pattern often seen when a person has had lung infection in the distant past (now inactive) caused by TB or a fungus. This interpretation would also be consistent with the finding of “calcified lymph nodes.” The first step that you and your doctors should take is to gather any and all previous chest X-rays you might have had, for side-by-side comparison with the current chest X-ray. Should these densities prove to have been present and unchanged, on X-rays taken more than two years ago, nothing more need by done at this time and “follow-up imaging” at 6-12 months would be a very reasonable approach.
If any other concerns are raised by your doctor or the radiologist, consultation with a lung specialist at this time might be appropriate, but I suspect that will not be necessary.

I too have been diagnosed with these granulomas They are normal caused by old scarring or from an infection earlier in life. Most the time they are harmless, but I have been told they want to keep an eye on them.
You should discuss with your dr and put your mind at ease.
