Need help

June 24 2009, I recieved unprotected oral sex from a sex worker in the US. Felt a ***** during the act and started feeling immediate discomfort. Within 8 hours felt burning sensation in the head of penis and noticed some swelling.
7/9 went to clinic for chlamydia, gonorrhea & syphillis test. Test came back all Negative. Took azithromycin.
Next week Felt burning in legs, lower abdomain, and head of Penis. Tip of Penis looked red and swollen. Felt sharp pain inside of penis in the middle of the shaft. Felt sick during the week. Scrotum has been tingling consistently since the first week.
7/30 went to clinic and was seen by a MD. She said it did not look like any STD.
8/12 went to clinic. Having burning pain at tip of Penis and Head of Penis and Tingling sensation on scrotum. Nurse said that it did not look like STD.
8/21 has blood test done for HSV 1, HSV 2, and HIV from an online company. Negative for HSV 2 and HIV. Positive for HSV 1 Representative said my antibodies were so high that it had to be from when I was a kid. (I did have cold sores as a kid).
8/26 went back to clinic - same symptoms - same result.
9/3/09 I went to a new family MD and told him my story. After examination he said that I did not have a STD. We ran another set of blood test with same result . Negative for chlamydia, gonorrhea, syphillis, HSV 2, and HIV(aids). Positive for HSV 1 AB, IGG 4.9. I have not had the classic outbreak of blisters on my penis or butt.
I have a burning red rash that seems to move from the tip of my penis, to my penis head, to just under lower side of my penis head on the shaft, to the skin on the right side of my penis shaft, to the skin on my scrotum,to the skin on the left side of my penis shaft and then back to the tip of my penis. This has been a consistant occurance. It never blisters but it gets very hot, sometimes itchy and feels like a sunburn. It last 3-4 days then moves to the next spot.

Is it possible you have HPV?  Yes, of course; at least 80% of sexually active people acquire HPV one time or another, and at any point in time 20-30% of sexually active people have genital HPV infections.  However, HPV does not cause any of the symptoms you describe and is definitely not hte problem here.

If you have eczema elsewhere, of course it is possible it could involve the genitals.  Almost any condition that causes rash sometimes affects the genitals.  If this is the explanation, a dermatologist would easily be able to diagnose it.

Hepatitis is not a likely explanation either -- but if your abnormal liver function tests have not bee carefully evaluated, that of course should be done.

I reiterate my conclusion above that your symptoms are not due to any STD or anything else you caught during your sexual encounter 6 months ago.  And that's as far as this forum goes; I have no advice about non-STD health conditions.  Continue to work with your own doctors on it.  But I won't have any more comments or advice.  Good luck.

More madness
Is it possible that I have some type of HPV? Skin around urethea seems altered somewhat.Never any burning while urinating and no discharge.
I have eczema on back and chest , could this be it?
Went to existing family doctor 9/13  - He said I was pre-diabetic?
Liver bloodwork off.  AST/SGOT 92  ALT/SPGT 131  alkaline phosphates 239, what are the chances of Hepatitis?
Can my mind really do this to me?
still seeking after almost 6 months...

Welcome to the STD forum.  I'll try to help.  But I suspect you will view my opinion as both good news and bad news.

The good news:  Clearly you have no STD.  Your symptoms are not those of any known STD; extensive testing has been negative for STDs; and if I have counted correctly, at least 3 clinicians (maybe 4 or more?) have assured you that you have no STD.  I agree with their assessments.  You don't mention herpes as a specific concern, but your statement about "no classic blisters" implies herpes might be on your mind. However, herpes simply doesn't cause either the specific symptoms you describe or, equally important, the pattern of symptoms -- i.e., their continuous nature, location, etc.

The bad news:  I cannot explain your symptoms.  Your description is consistent with magnification of normal body sensations due to anxiety or other emotional reaction to the sexual exposure.  That may or may not explain the situation; you and your doctor(s) are in a much better position to judge it than I can.  However, other than that possibility, this clearly is unrelated to the sexual exposure last June.

Continue to work with your doctor about possible explanations and treatments.  But you can be certain you have nothing that will ever cause harm to your health or that of your current or future sex partner(s).

Good luck--  HHH, MD
