Continuing STD or anxiety

Good afternoon, late last year i had two instances of unprotected oral from CSW pus others of protected. Developed a couple of instances of burning urine which foolishly ignored and later diagnosed with epididymitis by my GP after aching left testicle. Was presc Cipro for 11 days - 500mg twice daily. During cipro treatment i had an STD test done  - swab, bloods and urine sample, all negative. I also waited approx 3 weeks after finishing cipro to be retested and again negative results. Long term partner also tested and negative as i was terried i has passed something to her

Questions are -

1) Can i rely on the 2nd set of results. I did do at the time and had unprot sex with my long term partner after these results. I am now worried that they may have been continuing to mask std and that i could have passed something on to her, after our tests.I took two 4 packs of chlam and gon treatment later on because of my anxiety over this, but this will not help my partner.

2) I still experience what i thought was discharge after unrinating but tense myself up so much that this may be drips after as i do not complete full urination. Also continue to milk my penis to any discharge - only light mucus in there which doctor said was natural. I did have a clear watery discharge other night but possibly after erections in the night. Is this anxiety? My anxiety has been 9/10 no the scale since the incidents

3) Semen since infection last year now a little thinner and clearer than it was and a get dull ache in testicles after masturbating - what is this?

Welcome to the forum.  Thanks for your question.

Can you say more about the epididymitis?  Detailed symptoms? Urethral discharge? Results of STD testing before taking ciprofloxacin?  How about urine testing for non-STD infections?  Also tell me more about how quickly the symptoms started to improve after starting cipro.  That antibiotic is not recommended for any STD, or for epididymitis from gonorrhea or chlamydia.  I'll also add that sexually acquired epididymitis is rare after oral sex, if it occur as all; and there is no known STD epididymitis other than gonorrhea and chlamydia.  Many doctors over diagnose or over treat the possibility of epididymitis in men with testicular pain.

1) If you had negative tests for gonorrhea and chlamydia at least 2 weeks after your last dose of ciprofloxacin, you can be confident those results were accurate.  However, if you were not tested before being treated, at this point there is no way you can ever know for sure whether you had one of these STDs at the outset.  Fill in the questions I've asked above and I may be able to make an educated guess.

2,3) These continuing symptoms sound like increased bladder tension, which can result from a prostate problem or, most commonly, as a result of anxiety  or other emotional tension.

One possible consideration is the male chronic pelvic pain syndrome, or CPPS.  I suggest you google that term (spell it out) and start your reading with the excellent Wikipedia article.  Also see information provided by the Stanford University Dept of Urology.  My guess is you'll find many of the symptoms are pretty similar to what you have had.

In any case, it is very unlikely you have any STD at this point, and perhaps never did, and I doubt you infected your regular partner.

I hope these comments have been helpful.  Best wishes--  HHH, MD

Thanks for clarifying your symptoms.  While your symptoms are consistent with epididymitis, they are not entirely typical, and if you had that, I doubt it was due to an STD or that it was caused by the sexual exposures described.  Among other things, STD related epididymitis (gonorrhea, chlamydia) usually causes obvious penile discharge; the burning on urination they cause is continuous, not intermittent; and neither of these would be likely to relapse after initial success with Augmentin.  This might have been some nonspecific inflammation associated with your varicocele -- although that's just a guess.

At this stage, I recommend following your doctors' advice and moving forward without worry about STD.  As discussed above, I am confident you have nothing that will be harmful in the long run, either for you or your wife.

Hi there thanks for coming back to me so quickly - i actually didnt have an STD test before the cipro unfortunately only one during it and the 2nd the 3 weeks after. The buring urination occured a handful of times, once in the day but mainly morning and night for a few days and no discharge whatsoever. The epididymitis symptoms were a very warm left testicle, sore tube behind it and very uncomfortable to sit and generally achy left testicle kind of like toothache. Initially the GP put me on a course of augmentin which i think is a less strong antibiotic. This seemed to work but after completion it came back and i was put on the cipro by a private gp instead.
