Lacunar infarcts are a pretty elementary diagnosis on an MRI. There's a few things they can conceivably be confused with, but that is unlikely to happen. Those types of infarcts can occur and be completely asymptomatic. In 25 year old people they could have occurred in utero (especially if they are periventricular) , or as a result of heart valve disease, or as in older people, hypertension. Anectdotal, but adviseable - do not ingest large amounts of vitamin E (tocopherol).
