lab results

If the T4 is 0.49 & the TSH is18.451 almost 3 months after a radioactive iodine treatment what does this mean?

You have become HYPOthyroid after RAI and need to start levothyroxine.  This means the treatment was successful in taking care of the hyperthyroidism, but now the trade-off is hypothyroidism that needs treatment (this is very common).

It means that the RAI worked by rendering you hypothyroid and thyroid meditation is now needed, assuming you are not on them already.  Thyroid medication will bring down the TSH and up the FT-4. In time they will be in Labs reference range, although it might take some dose adjustments until the RAI has totally completed its job.  In some cases the thyroid dies slowly and as it dies thyroid medication dose will need to be adjusted accordingly to compensate.
My TSH went to 29 approx three months after RAI, in which I was put on levoxyl 100.
