I was told I now have a goiter!

I am having a ultrasound in the morning,  I have the symptoms of being tired all the time, dry brittle nails, thinning hair, achy muscles and joints, unexplained increased weight (40 pounds in 7 months) and low to no libido.  My last TSH was Dec 2011 and was .78.  No T3 or T4 was done at that time.  In Feb 2011 my TSH was 1.65 free T3 2.5 free t4 0.98 total T4 6.1 November 2010 no TSH was done, free T3 2.4 Free T4 1.09 total T4 7.3 TPO 193.  The highest my TSH has ever been was 1.193 in dec 2009.  I am Diabetic, have low b12, anemia, and low calcium.  I take vitamins to keep the anemia, b12 and low calcium in check.  I have had to be treated for osteoporosis.  I am 45 years old.  I have also had a gastric bypass 20+ years ago.  November 2011 had hysterectomy.  I am on very low dose HRT.  I feel absolutly run down.  Although I am tired, I do not sleep well.  Does this sound like hypothyroid?

You have euthyroid hashimoto's at this point -- ie, normal thyroid function but with evidence of an autoimmune attack on the thyroid (positive TPO) -- this increases the risk of fuutre thyroid dysfunction but the current TSH levels suggest that treatment of the thyroid is not likely to be helpful.

I just found out that my TPO is 387.  When should my Endo start thinking about treatment?

My thyroid scan says normal sized thyroid lobes with hetergenous parenchyma.  No focal replacing lesion demonstrated.  Labs drawn 6/16/12 total chol 228 (I am mostly a vegetarian) Glucose 79 (very well controled diabetic) TSH 1.61 (.4-4.50) free T4 1.1 (.8-1.8) total t4 7.0 (4.5-12.0) total t3 83 (76-181) WBC 16.2 (3.8-10.8) Absolute neutrophils 13657 (1500-7800).  I am still waiting on thyroglobulin antibodies and TPO.  UA was WNL.

also under treatment for anemia, low vit D, Low B12 all managed with supplaments.  Also taking Prolia Q 6months for low bone density.  I am 47 Year old Female.

As far as increased wbc's no fever, or other symptoms of infection.  No ESR or CRP was performed.
