secondary hyperparathyroidism

Dr Lupo,

I am currently being tested for secondary hyperparathyroidism.  I have suffered with c/o kidney stones off and on for as long as I can remember.  For about the last three years I have week long spells of extreme fatigue, lethargy, pain up and down my whole right side of my body, dizziness (feel like I am going to pass out), nausea, horrible lower back and sciatic pain, weakness, and painful gas and bloating.  I can go from feeling really good to bed ridden within 24 hrs.  Until recently I thought the spells may have something to do with the stones.  So I just muddled thru with lots of water, and ibuprofin.  Two years ago I was tested for secondary hyperparathyroidism and my pth was high, and I showed a significant vitamin D defieciency.  In March of 2007 I had surgery to remove a massive amount of stones in my right kidney.  I had a stent left in for a week after the surgery.  The urologists was surprised to find so many stones because they did not show up on the x-ray or cat scan.  There were so many that he ran out of time to get them all.  During my follow up with the Urologist he advised me to take caltrate.  Caltrate made me feel sick.  I decided to go see a nephrologist, who tested my pth again and it was in normal range.  He advised me to take hydrachlorathyazide which also made me feel sick (similar to the above symptoms).  In January 08 I passed a 7 mm c/o stone from my left kidney.  I am suppose to do another 24 hr urine and a blood panel for the Nephrologist in April, but am in the middle of a spell.  My primary care sent me to have my pth and vitamin D checked today.  My primary care also had an ultra sound and hidra scan done of my gallbladder because of my upper abdominal pain.  Both came back fine, but this spell I am in got worse from the moment they injected me with CCK?  She is really trying to get to the bottom of what is going on, but in the meantime I am miserable.  I am a very active Mother of 4, I am 36 years old, and when these spells happen my whole world stops.  What are your thoughts on this matter?

Without any labs, it is difficult to comment.
With the kidney stones and low vit D my first thought is PRIMARY hyperparathyroidism -- which causes a low 25-OH-Vit D, high PTH and high Calcium level.
