Old medical records from a hospital that closed.
I am trying to track down my 1980 medical records from Mt Sinai Medical Center Cleveland. The hospital sold to PHS in 1996. Primary Health Systems went bankrupt in 2000. I trying to file VA disability claim and need medical
proof of my chronic condition.
