If kidney, heart and liver problems have been taken care of, then the pain and swelling in the legs could be due to varicose veins with faulty valves of the deep vein causing venous pooling. A dopplar study or angiogram of the blood vessels of the leg should be done. Wearing a hose or stocking specially designed for varicose veins may help.

Did they test your sodium level in your blood or a 24 hour urine test for sodium? What about testing your thyroid function & ADH or antidiuretic hormone, which helps regulate water in your body?
Did they do a color doppler on your legs to test circulation? Peripheral vascular disease can affect extremities with pain and swelling. My grandma had PVD. Your heart's okay you say, but what about your triglycerides and cholesterol levels?
Symptom checker lists besides peripheral vascular disease, arthritis, hypothyroidism and obesity as causes for pain and swelling in one's feet. And they say emphysema can cause swollen legs and feet & list pain as a symptom. It lists deep venous thrombosis as giving calf pain & swelling in feet.
