uncontrolled B/P

Although I'm on Norvasc, Metoprolol, Clonodine my B/P is never in the normal range. Lately it's been running around 240/120 most all the time, especially in the evenings and nighttime. I had a stroke in 2006 and have been on Lasix 80mg. 3 times a day, Demedex 20mg. 2 times a day. My dr. doesn't give me a reason for having to be on such high doses of diuretics or reasons for my B/P being so high although I'm on all those B/P meds. I asked him once if I had Congestive Heart Failure but he said no, that's the one thing I don't have..but didn't go any further in to why all the diuretics..I do know that if I don't take them I swell all over, especially my right leg and foot and my hands and face. Do other people have this problem and if so, what should the diagnosis be for all this? By the way, I'm a 62 yr. old female.

Every so often (this is not common) out-of-control blood pressure is due to a neurological disorder or disease. The reasons and mechanisms for this problem are unclear. Essentially the vasculature inappropriately contract. This kind of a problem cannot be easily resolved. If you can afford to do so, attempt to get a second opinion.

Blood pressure of 240/120 is unaceptably high. Generally any number over 200 greatly increased the probability of a blood vessel rupturing. The diuretics reduce fluid volume (called body capacitance). Abnormally high blood pressure is sometimes due to a malfunctioning of the sensors that provide feedback to the musculature surrounding the major blood vessels.. Usually this is in the right and left carotids, but difficult to resolve.

Ask your physician about the advisability of a transdermal nitroglycerine patch. This will dilate the blood vessels, but the body tends to be acustomed to the medication. In the meantime take timed-release niacin (which dilates blood vessels) and the supplement Argenine, which also opens blood vessels.

Pay meticulous attention to salt intake and take your own pressure regularly, always on the same arm (the left is the ususal) and note down the pulse rate, as well.

The etiology of uncontrolled high blood pressure is not always clear, even to the best physicians. If you can, I would seek a second opinion from a team at a teaching hospital.

Sorry to hear that you have had a stroke.

The reason for the diuretics is not only to reduce the oedema, but also to bring down the high blood pressure.

As the medications that you are presently on do not seem to be bringing down your blood pressure, it would be worth asking your doctor to consider changing the blood pressure medication.

Norvasc is a calcium channel blocker and is used to lower high blood pressure.

Metoprolol is also used to lower high blood pressure.  Regarding this drug, ask your pharmacist about whether this drug has been recalled.

Wockhardt announced a recall of 109,744 bottles of Metoprolol Succinate Extended-Release 50mg Tablets, the generic version of AstraZeneca's Toprol XL.

This recall was initiated due to a dissolution test failure observed at the nine month time point. Dissolution tests verify the time taken for the active ingredient to release into the body, and help predict how the drug performs inside the body.

The affected 50mg tablets are supplied in 30-count bottles and have Lot # LN10686, LN10687, LN10688, LN10707, LN10708 with an expiration date of 02/15. The recalled lots were manufactured at a Wockhardt plant in Mumbai, India.

You do not say which Metoprolol you are taking, but it is still worth checking this out.

Clonodine is also used to treat hypertension (high blood pressure) and ADHD.

Lasix is a diuretic which is used with people who have congestive heart failure.  As the doctor has said that you have no heart problem, I am surprised that you have been prescribed with this type of diuretic.  

Demadex is also a diuretic.

If you have not had a blood test taken to check your kidney and liver function, then it would be a good idea to ask your doctor to get this done.  This should be done routinely at least once a year with all of those meds that you are taking.

To help you to get your blood pressure come down, it would help you to do relaxation techniques, breathing techniques to help you relax too.  If you do have a salty diet, cutting out salt, will also help.  If you eat preserves, like pickles these can spike up the salt in your body causing high blood pressure.

Gentle exercise will also help.  No doubt you may have been already given advice from your physiotherapist when you had your stroke as regards what type of exercises to do and how often.  

You do not say what type of foods and drinks you consume.  But if you do make some changes to your diet by cutting out or drastically cutting down on salt and sugar, this may be helpful to bring down the high blood pressure.

I would suggest you make an appointment to see your own doctor or another doctor at the same practice and voice your concerns with regard all these medications that you are taking and that they are not helping to reduce your high blood pressure.  

It may be that an Ace Inhibitor may suit you better than the Calcium Channel Blockers, or the dosages may need to be adjusted.  Make sure you take your meds as prescribed and at the right times of the day.

Talk to your doctor about referring you to a dietician to talk about a healthy life style eating plan and about the types of exercises that will be suitable for you.  Swimming and exercising in the swimming pool is a good form of exercise to tone up your muscles if you are not able to do much walking.

You may find also that meditation to help you to calm down and relax helpful too.  Do not take your blood pressure just after you have been rushing about, or after you have drunk a hot drink.  Only taken your reading when you are relaxed.  If there is any stress at home (or work), try and avoid any stressors.  Stress, worry and anxiety can also send your blood pressure to rocket.
