I had a colonoscopy and a following tansanal excision surgery to remove an anal mass, just now I received the bioposy results from lab, which I desperately need some help to understand how severe it is,
'High grade squamous intraepithelial lesion (HSIL; AIN-2) arising in a background of condyloma acuminatum with low grade squamous epithelial lesion (LSIL; AIN1).'
'Immunohistochemical stain to p16 fails to show diffuse strong staining.'
My questions are,
1.The shape of the mass descriped by the surgeon, 'distinct frond-like and slightly fungating', so is it a wart with the base classified as 'LSIL AIN1' while the tip of the wart is 'HSIL AIN-2'? Is this neoplasia grading normal for an anogenital wart leff without treatment for years? How serious is it in terms of precancer/cancer malignancy?
2.Can I interpret the p16 result as 'negative'? Is it for the HSIL or the LSIL?Or the entirety? If so the overall pathology shows no cancer progression?Will I get a downgrading to LSIL or negative after the surgery and proper recovery?
3.The surgeon stated in a post-surgery summary, 'the mass can be removed in the submucosal space without a full-thickness excision', is this procedure accepatble now considering the presence of HSIL?What about the recurrence chances after this procedure?
4.I don't think anal pap smear test for high risk lesions/displasia is very common or widely prescribed for anal cancer prevention, how should I proceed with my follow-up checks after this procedure?
Pardon my lengthy questions, I've been frustrated for months since I found blood in stool, and now even more anxious after seeing the biopsy, your inputs and help will help me tremendously and I thank you in advance!
