By cognition problem I meant something more like budding Alzheimer's. I'm not a doctor, I can't make a diagnosis. Being sad sounds like depression, but talking to oneself, unless she's hearing voices (see above post) isn't necessarily a terrible thing. A psychologist can make this diagnosis.

Yes , there has been huge shift in her personality . She remains sad , has complains which are due to obvious reasons . Could you elaborate the cognition problem

Is she unhappy? Having a hard time? Not able to function in important ways? If so, there might be a cognition problem. If not, it might just be a quirk of her personality.
