What is this called?

Most of the time, nearly all the time, when I try to fall asleep I wake up as soon as my consciousness starts to slip away in a small jerk, or spasm, I kind of just, wake back up in a painful sort of way...

it honestly feels like my subconscious doesn't want me unconscious, it just... feels pretty terrible.

Now my question is, Does that kind of problem have a name?
like a single word, or condition, that aren't what I'm about to list.

Now I have a history of;
Depression
Anxiety-attacks
and PTSD

These are probably contributing to the poor sleeping conditions, but that's not what I'm wondering.

I'm wondering if there is a word for the condition of not being able to sleep, or being suddenly forced out of sleep, or synonyms there of.

I get this too. I notice that when my anxiety is really bad it happens to me more frequently. I'm not sure if it is the same as what RonL is describing, I know that sense of falling, that usually happens with a leg jerk or something like that. What I experience is a little different, it is as if you are almost semiconscious and right  on the cusp of falling asleep but then feel yourself drifting into unconsciousness, but then a surge of adrenaline will radiate through our body.     It seems as if there is no jerk involved... or at least my husband never seemed to notice. Anyhow, to get to your question, I can't seem to find a name for what I try to describe except for the ones mentioned above. So I'm not sure if they are the same, and I've just been describing them differently.

Izzy, I have good news for you. What you're describing is very normal, very common, and very benign. There are different names for it: hypnic jerks, hynagogic jerks, myoclonic jerks, myoclonus, etc. I've experienced it many times. Sometimes people get the sudden feeling that they're falling, which causes them to awake with a startle. It's just one of those weird things the human body does that you don't need to be concerned about.
