In the simple terms, a breast mass (tumor or lesion) which has some abnormal features had been identified (seen), as well as some enlarged/suspicious lymph nodes.
The BI-RAD 4 rating means that these are suspicious findings (have an abnormal appearance), and the radiologist has recommended that the breast mass and the largest lymph node be biopsied, to determine whether or not they are malignant (cancer).
There is reason to be concerned, but they won't know for certain what these are until the pathology report from the biopsy procedure is completed.

The report indicates that a mass was found that does not have round or smooth edges. Often masses of this type do raise suspicion and as indicated a biopsy is recommended to define the true nature of this mass. I would think that your next move would be an appointment with a Surgeon to decide on the best type of biopsy for your particular situation. I think we always worry when something is found that is abnormal until we find out if this finding is benign or malignant. Regards .....
