ct scan results

my mother in law recently had a ct scan of the chest,abdomen and pelvis without contrast. and it says indiction: restaging small cell lung cancer...im just going to type what most of it says. just need someone to put it into terms not doc jargon...

findings in the chest: remains signigicant abnormal soft tissue involving the left aortopulmonary window, the subcarinal region, the left hilum, and the precarinal region. stable to slightly more prominent compared to previous scan. there appears to be further encasement of the descending thoracic aorta and also now extension of the soft tissue in the precarinal region up into the right paratracheal region. there is also new mild narrowing of the bronchus as it courses through the lymphadenopathy within the axilla. a new finding is a small pericardial effusion.

abdomen findings there is a mass within the right adrenal gland which measures 2.2 x 2.2 cm .this does not demonstrate hounsfield units of and adenoma and is highly concerning for metastatic disease. there is also loss of fat plane with this mass in the adjacent inferior vena cava suggesting invasion.

pelvis findings: there is diverticulosis of the colon.



im just needing someone to explain where these areas are located and what it is meaning..she will be starting a 3rd chemo next week.

It is impossible to predict the outcome of individual cases, and I probably cannot be more specific than merely repeating that the report does not look good, and that the prognosis is not good.

Hi,
The report is not very good.
It was done for re-staging (checking response to treatment) of small cell lung cancer. It shows that most of the initial lesions are persisting, and a few new ones have appeared.
The regions described in the report are located in the central part of the chest called the mediastinum, as well as in a major artery called the aorta, and bronchus (part of lung). The right adrenal gland (this lies just above the kidney) is also now involved.
Your mother-in-law's treating oncologist may review this report and perhaps shift to another combination of chemotherapy drugs.

Hi,
The pain may well be due to the arthritis. TB of the bone or the lung would probably be symptomatic with weight loss, fever etc.
Vitamin D is also known to cause sclerotic lesions of the bone. For a discussion on these bony lesions, and a list of possible causes, you may visit this site:

Thank you for responding to my inquiry. I am not familiar with having a blood smear done but will inform my dr. upon his return.  I have had extensive labs done and I have had elevation of RBCs,  My Rheumatoid factors are high one was over 100 and vitamin D levels were low I was placed on 50,000 units twice weekly, also Im having trouble regulating blood sugar and HTN even with meds.  I will be seeing a Rheumatologist in 1 week, I am praying some symptoms are due to arthritis. Is there any questions I should be asking him? Also I neglected to inform you that I had been exposed to TB 15yrs ago and because of my cirrhosis coud not tolerate the INH , could these rib lesions and denseness also be because of any of these factors.  I was never worked up for the possibility of TB.  Maybe because I have no cough and a normal temperature

Hi,
With chest pain and CT showing some lesions in the ribs, you need to undergo more tests to diagnosis the problem.
A PET-CT with FDG and F18 (for bone scintigraphy) would give us valuable clues. Alternatively, a regular bone scan may be done.
Whether the rib lesions are secondary to your previous health issues or a new development is difficult to decide just yet.
Have you had a peripheral blood smear done recently? Were the reports normal?

my complaint was chest pains. Ct. scan report states residual thymic tissue anterior mediastium. Mild prominence transverse thoracic aorta 2.8cm width. thickening of adrenal glands may represent adrenal adenomas or adenomatous changes. Suspected exotosis of rt. lateral 6th rib. Posterior ribs appear dense wth irregularity of cortices, sclerotic lesions cannot be excluded Bone scintigraphy may be performed to further assess.  I have extensive medical history, past history hep A,B C, cirrhosis 4th stage and fatty enlarged liver, Diabetes, HTN, Pancreatitis, Heart Palps, and a polyp was found on my gall bladder in 2007.  I also have unexplained enlarged lymph nodes in neck and thyroid and adrenal (neck biopsy was neg) My mamo,pap,colonoscopy,endoscopy all done this year was neg.  My dr. is on vacation and I would really like to know if any of the above findings should be of great concern and for what reason. Im a little suspicious about the ribs and what that could mean. Need I worry?

thank you for explaining this too me. I do have another question and I know the answer would be your opinion. Her doctor told her that she needs to sit down with her family and discuss what she wants done when she becomes unable to make her decisions on cont chemo. And she has lost so much weight she's down to 96lbs and the doctor was concern about until this report and said that isn't a big concern that the cancer is his main concern. I guess i'm asking from this report what do think or feel how much time she has left.I should say with patients with this type of cancer and report how much time do they usually have left because i know each person its different..thank you very much
