Adolescent Child Age 12

My wife was very disturbed after this situation, whichhappened at least 3 times. Once happened when my step son, age 12,  stayed for the weekend. It happened  twice when he slept over  at a close friend's place. The lady friend told my wife after the incident.
In all occurences, he was alone with her mom or this lady ( very close friend). He exposed himself (his penis) and he claimed he needed to go to the bathroom.
The lady did not speak to him about his wrongful act, and just asked him to go to washroom.
My wife had discussed with him right away after the 1st incident and he promised this would not happen again.
The other two occurances were  after the disucssion .
My wife, knowing what happened again, asked him if he was keeping his promise. He denied and he accused her mom not trusting him.

My wife is extremly concerned and is really at lose, not knowing how to deal with the situation.  Her belief  is this problem only happened when he was alone with her and her close friend, who is also liked a 'motherly figure' to her
son. He would not have attempted this act at school or anywhere else, otherwise he would have been in mega trouble already.

Two problems here, exposing and somewhat lying ...
This 12 year-old boy is a top student in school and he always strives for the best in any area.
Really need your  professional advice.

It seems like a bad idea to leave him alone with her after the first incident , what exactly happened prior to the behavior, did you ask your son, what happened to make him do that.. what was the situation like in the room where it took place.ask him pertinent questions, what did the friend say was happening immediatly before it? Whilst you are trying to get answers to this it would not be good to let him be alone in company till you have more answers.
