I don't know the health care system in Canada, so I don't know the implications of the doctor refusing to refer your son for counseling. What would the doctor say are his reasons for not being involved?
And what about the school and school board? Not that their perspective is correct, I'd just be curious about what their perspective is in not helping your son, and why they would send him to the office for a true "hug". It sounds like they have a different viewpoint.

I can understand your concerns. But I am having trouble following the years of problems. Sounds like he will be in 3rd grade next year? And now he is the bully and doesn't do his homework, etc.
I think if you could focus on just this year. What his teachers are complaining about. How are his grades? Perhaps what he does at home that bothers you. Then we might have a better picture of what is going on. The past is important, but we need to understand the present first.
Oh, and the other kid who used to bully him - that is still going on? What grade is that child in now?
I am sorry this is happening. Is his teacher entirely unsympathetic to your problems?
