Am I facing my final days?

I'm 27 male and get married in two months.  I've cried alot around my fiancee thinkining I'm going to die soon.

My whole life I've eaten bad.  Well month or so I've made a quick change and have eaDB actuallyn right. Switched from high carb to high healthy fat and protien diet.  And lost 25 pounds.  All was right in world. Got blood work and my ast was 68 and alt was 134. Albumin was slightly high. Bilirubum was normal.  Was slightly worried but as I lost weight **** hit the fan.

I started getting sharp excruciating pain in upper right near rib cage that sometimes shoots to left.
On top of that been very itchy(but I have simple partial sizures and having alot of floaters so could be that as I've been itchy as a kid)

Also been super fatigue.

I thought gallbladder. But got ultrasound and doctor says no sign of abnormal gallbladder.  But showed fatty liver and spleen.  This is where I freaked out.  Itchy, fatigue, sharp pain, and normally enlarged spleen is end game.

Doctor has to wait for monday for blood work.  I'm terrfied and my fiancee and me have been crying. I don't want to die im doing every thing I can to get healthy.  

I was wondering if my worry is founded or is the symptoms not as bad as it sounds?  I'm trying to convince myself that at 27 and a non drinker that I don't have cirorsis.

Hi if you have fatty liver you doctor will likely recommend weight loss to get to a normal weight.

I am only a lay person who had hep c for 37 years which caused liver cirrhosis after being infected for 30 years. I have had liver cirrhosis for 10 years now my health should not get any worse and my liver may improve some with time since I am now cured of hep c.

Elevated liver enzymes only indicate something is irritating your liver it does not relate to how much damage you may have. Given your age I am assuming you likely do not have any or only a little damage but this needs to be determined by your doctor. But even if you should have higher amounts of liver damage the liver is a remarkable organ capable of self repair if not too much damage has occurred.

Even liver cirrhosis is not a death sentence if you can work to minimize additional damage. At this point I suggest you stop all alcohol and use care with Tylenol ensuring you do not take more than the daily recommended dosages

I really think you will be fine. See your doctor and follow their recommendations. With proper diet and care you can live a normal life span but you will need to get a complete diagnosis about what is causing your elevated liver enzymes to know for sure.

Thank you for taking time to reply.  Don't drink like at all.  I've eaten cheeseburgers as staple of my diet for many years.  I've read many story's like yours and gives me some hope.  I didn't get this way over night but the sharp pain in tight side and the very enlarged liver has me super in dispair.  To point I asked my fiancee to not marry me because I don't want her to be a widow. Fatigue, enlarged spleen, itching, sharp pain. Its all so real.  Funny thing is the pain wasn't there until I lost weight.
I know because I got my ultrasound results back. He said gallblafder was fine but a enlarged liver and spleen. I have middle back pain.  I read that a enlarged liver could push agiasnt the floating rib and cause that pain and back pain. Liver doesn't feel pain so the sharp pain I feel isn't the liver itself?  That's a relief but enlarged spleen doesn't make me feel good if its associate with cirrorisis.

As far as relationship we are not going call it off but just hate for to have to start a marriage off like that.
I have had an enlarged liver and spleen for decades. So far you are not dying yet. What has your doctor said?

“Causes of an Enlarged Spleen. An enlarged spleen can be caused by infections, cirrhosis and other liver diseases, blood diseases characterized by abnormal blood cells, problems with the lymph system, or other conditions“

Could be a lot of things other than cirrhosis

Could be a lymph system issue which is likely very treatable
My doctor said going wait for blood work to determine to do fibroscan or biopsy.  I feel like fibroscan would be better option... I can't afford a biopsy and heard fibroscan is almost as accurate.
Liver biopsy is still considered the gold standard. But it is an invasive procedure having a sample of liver being taken with a biopsy needle. A fibroscan is not invasive requires no special prep but I know some people suggest having it done while fasting. It is most accurate with no damage or serious damage and less accurate in mid ranges.

My pre treatment Fibroscan was 27 and my 2 year post Fibroscan was 33 which is actually worse but I feel they are more attributayable to operation variation and not clinically significant.

I have had 4 liver biopsies, one every 5 years of my follow up with my doctor while I was being monitored after being diagnosed with hep c.

Your insurance won’t cover a biopsy? There is also a blood test called a fibrosure but it has similar limitations as the Fibroscan.
Imbsure it will but at 70% and after hit deductible which would make it a 2000$ thing at least which is alot to someone who only makes 10$ a hour and lives paycheck to paycheck
Have you contacted you insurance to confirm the estimate but yeah that sure makes a good argument against biopsy other than the whole needle in the side thing.

While ultrasounds are great at confirming cirrhosis mine do say “coarsened echotexture consistant with existing diagnosis of cirrhosis” so there would at least be a mention of the surface of the liver looking rough and uneven as a healthy liver is smooth.

I am betting your ultrasound says nothing about a coarsened surface texture.

Nonalcoholic Fatty Liver Disease (NAFLD)

Like the name says, alcohol isn’t involved in this condition. There are different types of NAFLD.

Simple fatty liver means you have fat in your liver, but you may not have any inflammation in your liver or damage to your liver cells. It usually doesn’t get worse or cause problems with your liver. Most people with NAFLD have simple fatty liver.

NAFLD is more likely if:

You’re overweight or obese
Your body doesn’t respond to insulin as it should (called insulin resistance)
You have high levels of triglycerides or “bad” (LDL) cholesterol, or low levels of “good” (HDL) cholesterol
You have type 2 diabetes
You have metabolic syndrome. This is a mix of conditions that make you more likely to get type 2 diabetes and heart disease.

For NAFLD, weight loss is one of the best treatments. Weight loss helps reduce fat, inflammation, and scarring in your liver. Losing just 3% to 5% of your body weight can cut down on how much fat is in your liver.

What is Wierd is pain starter after I lost twenty pounds.  Wonder when you lose and burn fat if liver has to filter it?
No idea like I said liver doesn’t really feel pain. Have you been exercising as part of your weight loss plan? Maybe you strained something

Your symptoms of enlarged spleen and liver can both be explained by fatty liver

“Some of the most common causes of an enlarged spleen include the following: liver disease (cirrhosis due to chronic hepatitis B, chronic hepatitis C, fatty liver, long standing alcohol abuse); ... Inflammatory disease (lupus, rheumatoid arthritis)”

Ya that is what scared me.  I read enlarged spleen sign of cirorsis and with pain and fatigue got scared.  But sounds like regular fatty liver can cause enlarged spleen.
That is correct don’t jump to the worst case scenario the fatty liver is the most likely cause at this point without more information.

How do you know you have enlarged spleen and liver? The liver itself has no pain receptor nerves but every internal organ is surrounded by a membrane that does have pain receptor nerves. If the liver is very enlarged this could cause the liver to push against this membrane causing a pain sensation.

My liver is enlarged but I have never had pain in that area. Because I have portal hypertension as a result of cirrhosis I also have an enlarged spleen. People with enlarged spleens due to cirrhosis often have low platelet counts well below the minimum normal of 150. My platelet count was about 80 to 90 before I was cured of hep c but now is about 110 still low but improved.

Unless your doctor tells you some dire news which I really doubt I feel you are being very premature cutting of your relationship. Right now as best as I can tell you are more likely to die from the same rush’s others your age have generally accidental death and not from liver disease.

Don’t make any life altering decisions on some elevated liver enzyme test results and a fatty liver diagnosis. Wait until you have more information from your doctor and go from there.
Also with having liver cirrhosis for now 10 years I feel fine no issues except a low platelet count. No itching and all my other test results are normal. Because I don’t have elevated bilirubin I don’t have any jaundice (never have had jaundice) or any itching. Itching is a very vague symptom with many causes including stress.

I likely contracted hep c at 19 years old. Found out I was infected when I donated blood at age 32. I was diagnosed with cirrhosis at 50 I will be 60 this month.

I can’t say you will live forever but I am willing to bet you get as old as me and older.

Put down the cheeseburgers and lose some weight you should be fine. Of course you should wait for a proper diagnosis and prognosis from your doctor but that is my bet not being a doctor and not knowing anything about your medical situation beyond what you have posted.
